Warming Up/Muscle Spasms

in 2 weeks i’ve had 2 muscle spasms in 2 different parts of my back. 1st one i was doing power cleans and i caught a mean one in my lower back… so bad i had to go to the ER because i couldnt walk to the car alone. 2nd one i was doing T-bar rows and i didnt feel it until the next morning but it was in the left-lat area… i’m doin the movements with pretty heavy weight(for me) but my form is strict.

Maybe i’m just now warming the muscles enough beforehand? only thing that seems to help is my wife’s massages but it shouldnt hurt that fuckin much.
