Wow… sounds like you might be overtraining to the point that something in your body is telling you to slow down… by falling apart!
I recommend putting in a few lower intensity “active rest” days to give yourself some time to recuperate… from what I heard of your rep & set range I find it incredible that your body can take such punishment…
