Weighing Options Before Committing to TRT

So, I’m 26, 5’10", 235lbs (a bit heavy I know) and have been dealing with secondary hypogonadism for years. I’ve never tested above the 500’s, and my most recent bloods (2/13, 4/13, and 2/14) were 279, 405, and 313 respectively. So, my doc and I agreed to try a Clomid restart. He prescribed 50mg EOD for 60 days, wait 3 weeks after stopping, and get new bloods. My questions I was hoping could be answered are these:

  1. Would I be better off with 12.5mgs of Clomid every day, or 25mg? 50mg EOD seems like a lot but I could be wrong.
  2. Is three weeks enough time to wait after stopping the clomid to get new bloods?
  3. Thyroid - what all should I really be testing concerning thyroid when I get my new bloods? My tests have been a bit low in the past and want to make sure I test my thyroid thoroughly next time.

I’ve beat my pituitary to hell the past few years with stimulants and supplements, etc. so hopefully the clomid is the kickstart I need.

Any response would be greatly appreciated. I can post the last three blood work’s I have if needed.

I wouldn’t treat it like a restart. I would work it like normal treatment. Start with a dose that should absolutely work if it’s going to work at all. If test results are positive, reduce the dose for a couple weeks and test again. Repeat until you’re off clomid or you find a dose you have to stay on to get the desired results.

25mg ED or EOD produced the same response for me. Just did some BW after a couple of weeks at 25mg 2x weekly. If T is still good, I’ll go down to 12.5mg 2x weekly.

You should have your thyroid tested too, before committing to TRT. I`ve started treating my thyroid and my well-being and testosterone levels have increased dramatically.

Just my $0.02

Thanks for the reply’s gentlemen. Here’s what I’m going to do. I know I’m secondary, so I need to try a restart for my own sanity before committing. If that doesn’t work then I’ll reconsider my options at that time. So, I’m going to do:

25mg ED Clomid 3 weeks
25mg EOD Clomid 2 weeks
25mg 2x week Clomid 2 weeks

Does this schedule seem optimal or do changes need to be made? After I’m off the Clomid for 3-4 weeks, I’m going to do comprehensive bloodwork, namely:

Total test
Free test
Estradiol
LH
FSH
TSH
Total T4
Free T4
Total T3
Free T3

…those are big ones anyway. Do I need to add anything to that list?

Thanks again for the replies.

EDIT: Here are my last three bloodworks and their dates:

2/13:

TSH 2.60
TESTOSTERONE, TOTAL, LC/MS/MS 240 Range: 250-1100 ng/dL
TESTOSTERONE, FREE 44.1 Range: 46.0-224.0 pg/mL
TESTOSTERONE,BIOAVAILABLE 102.2 Range: 110.0-575.0 ng/dL
SEX HORMONE BINDING GLOBULIN 18 Range: 10-50 nmol/L

4/13:

LH 1.5
FSH 2.6
Testosterone Total 416
Prolactin 3.7

2/14:

Testosterone, Serum 311 Low
Free Testosterone(Direct) 8.2 Low
T4,Free(Direct) 1.31 ng/dL 0.82 - 1.77 01
TSH 1.850 uIU/mL 0.450 - 4.500 01
LH 2.7 mIU/mL 1.7 - 8.6 01
Estradiol 30.6 pg/mL 7.6 - 42.6 01
Prostate Specific Ag, Serum 0.6 ng/mL 0.0 - 4.0 01
Insulin-Like Growth Factor I 192 ng/mL 98 - 282 02
T3 Uptake 27 % 24 - 39 01

Does this help give some insight into what may be going on or should I continue with the clomid for a couple months, and get new bloods (the list at the top of the post).

Anybody?

Have you had bloodwork since the clomid ?

I haven’t started the clomid yet man, I was just wondering if anyone could tell from my last three bloods what the problem was for sure. I think I’ll just run the clomid, then get new bloods (including thyroid).

#1 - I would try 25 ED if no sides… then stay for about 2 months… do bloodwork
#2 - IMO NO
#3 - I cant help with this…

Your plan isn’t bad, but I would test at the end of each phase. At least after the 25mg EOD phase. You may need to stay on a low dose to keep T in the desired range. I would test for T, Free T, LH, FSH and E2. I would keep some aromasin on hand. Pretty sure you’re going to need it.

Thanks, I was going to test t3 and t4 as well, just to cover my bases.
