Some basic facts:
If you gain 1 lb of lean mass for every 1 lb of bodyfat you will become leaner.
If you gain significantly more lean mass than bodyfat you will be much leaner.
If your bodyfat % goes up 30%, you gained AT LEAST 30% more bodyfat.(though the actual BF% number doesn’t matter much)
Observations:
You don’t look leaner.
You say you’ve gained 3-4% bodyfat(up from 12)
You say you’ve gained 45 lbs.
Conclusion:
You gained mostly bodyfat.
You gained at least 30% more bodyfat than lean mass.
You gained at most 20 lbs of lean mass and at least 25 lbs of bodyfat.
Other impressions:
Some of your lean mass gains were probably water weight, so
you probably gained 10-15 lbs of “dry” muscle mass in ~25 weeks time.
In my opinion, this is near optimal in terms of muscle gain.
Could you have accomplished this with less bodyfat gain?
Probably.
Will you spend more time trying to retain mass while leaning down latter?
Probably.
Did you fail? Definitely not.
Suggestions:
You should slow down your weight gain by adding additional activity(cardio, walks, etc…) and once you’ve hit a wall with the time you have to train, you should draw down your calories and lean down through diet.
How long before you “Diet down” is going to be entirely dependent on you efforts to slow fat gains(if you decide to do so).

I see math is not your strong suit. If you gain 1 pound of muscle and 1 pound of fat, you increase your BF% unless you are already over 50% bf.

Wrong. If you weigh 180 at 20% bf and you gain 20 lbs, 10 of them are fat, 10 of them muscle. You started out with 36lbs of fat(20% of 180), you now have 46lbs of fat, but you weigh 200lbs. 46/200>36/180, so you increased your BF%.

Switch in cycles of wide stance squats if you feel your legs aren;t big enough or your squat poundages aren’t high enough. They will let your body adjust to higher loads and when you return to shoulder width stance, your back will be able to handle heavier weights.
Too much wide stance squatting MAY widen your hips more than a bit, and at your height you need as slim a waist as possible to maximie your V taper…but I wouldn;t worry about it tbh. If it becomes an issue later on, you can avoid it altogether.
