I broke mine almost identically to your friends. The X-ray in the pic looks very similar.
Pain killers that are narcotic in nature will not hamper healing, but anti-inflamatories can slow bone growth in a fracture.
My suggestions are, keep in in the sling and dont move it. He needs to learn to hold his shoulder back to keep the scapula from rotating forward while wearing the sling. Many will allow the shoulder to slump forward and this will not be very functionall once healed. I found sleeping in a reclining chair with my elbow propped up with a pillow a great help.
What the ortho’s told me about the break was that this kind of break heals well and is very strong because of the bridge created by the smaller middle piece seen on x-ray.
If healing is delayed there are bone stimulators used to try to get it healing faster. If that doesnt work there is always the plate and screws placed surgically.
How old is your friend, and what was the cause of the fracture?

The diabetis will slow healing down. He will have to be careful about his sugars.
If he was in his 30’s or 40’s I’d have suggested a T-booster like Alpha Male, but he’s 20 so he doesn’t have to worry about low T levels.
For a 20 year old the big problem is gonna be to not use the arm even though it is feeling better. 4 weeks out it wont hurt too bad and he will want to use it.
