Hey man how’s trt been going I just started on week 4

Its been good boss. Kind of an after thought anymore. I have been cruising at 200 mg per week but just dropped down to 120 mg to get bloodwork done for my doc. I have my 6 month check up on Febuary 7th. After that appointment I’m hiring a coach and plan on doing a show this year just for fun. Over all I’m feeling good. How bout you? How are things 4 weeks in?

Ive been taking metformin for 6 months but within the past week my coach has asked me to stop taking it so I did. He doesnt want me taking it if I dont have to. I just ordered dhea on amazon and I am going to start taking 25 mg a day starting today.

Joint improvements, some mental no energy or libido improvements yet idk why

I have varicocele and also all the same symptoms (low libido, ED, low sensativity, etc), I am currently on TRT and have “some” improvements but mainly mental and body composition wise. I have been on 8-9 months now and on daily injections. Everything is great, except libido and erections.

There is a lot of misinformation (and misunderstanding) regarding varicoceles, if you have a varicocele on the left side a lot of people assume it’s just the testicle and thus testosterone that is impacted. This isn’t true, I also believe the adrenal hormones go into a state of shock (over-synthesis) resulting in a down-regulation of thyroid function. This happens because the left testicle is connected to the left kidney/adrenal gland and the toxic metabolities that cause testicular atrophy (shrinkage) are actually adrenal hormones back flowing. The down regulation is subtle, but enough to cause low libido and ED issues. I would argue that reduced thyroid function (due to high cortisol) is what causes the shrinkage, due to a variety of factors.

I typically see high rt3 (due to high cortisol) and low ft4 (Which is required for erections, I will post studies in a bit…) in varicocele guys. TRT rarely fixes libido in these guys, because that isn’t the main system that has been messed up. Your thyroid labs may appear “normal” but you will have had alterations due to varicocele and TRT.

My FT4 essentially halved before and after getting a varicocele (mine was due to an injury).

Testosterone increase iodine re-uptake and a few other factors that increase T4/T3 conversion, this however has the side-effect of making things worse for varicocele guys who already have low (or subclinical) FT4.

So what is the answer to all of this? It’s complex, but I believe thyroid replacement is crucial along with TRT for varicocele sufferers.

Well if you block the veins via surgery why wouldent that fix the metabolite reflux and fix the adrenals?

Dealing with this now with a left varicocele that seems I developed after a vasectomy. Would stopping HCG and starting thyroid meds help?
Or am I better off having a surgical correction?

I wouldn’t get surgery, but I would get emobolization which is less invasive. Personally for me, I seen little improvement other than reduction in pain. I would get it done and see where my bloods were at 3 months later, if no improvement start pursing thyroid route.

Hi I responded to this thread maybe a year ago. I can not remember. I had bilateral varicocele grade 3 on the left and a grade 2 on the right. I rember when they first appeared. It caused me so much pain it drop me to me knees and I went to the emergency room and they told me I should see a urologist. I was 25 at the time and the urologist told me I should have them removed and I did not listen. Three years later that when I started experiencing low T symptoms. I am now 33 and have both varicose veins removed hoping that it would get rid of my symptoms but sadly to say it did not. I am still trying to figure all of this out.

My question to you is which thyroid homes should I really be ing looking in terms of a varicocele. And what else should consider monitoring.
