Uric Acid Levels

Don’t know what you have been prescribed. Here in the uk we get prescribed allopurinol which is the preventative tablet to stop gout flaring up. That I take 4 times a week. The other tablet is called diclofenac, which is a powerful anti inflammatory to bring down the swelling when gout flares up. I’ve never had a problem with allopurinol but diclofenac can cause stomach problems when taken long term, and i’ve suffered with them in the past when my gout was at it’s worst.

They haven’t told me which one the will give yet. Yea its probably different here In aus. Something to for me to keep in mind. Hopefully the preventative one works well.

If there anything like here in the the uk. You will probably get one a day of the preventative tablets to start. As time goes by you’ll work it out. Just like training really. Keep a track of what works and what doesn’t. I don’t drink or smoke anymore and try to keep my diet clean. I don’t get into the whole purine thing like my manager does. Good luck with it all. Gouts can be a nightmare but if you keep on top of it, no reason why you can’t keep doing what your doing.

Hey man, a physician here, I will give you my opinion, even when I’m not very fond of talking about med stuff here but I see u worried. Basically you can have hugh uric acid levels through 2 mechanisms (very logical)

  • Overproduction: excess of consumption (red meat, alcohol) +/- any genetic condition which alters anormal purine metabolism–> besides being diagnosed from a rare genetic anomaly, I would decrease red meat (and decrease your colon cancer risk as well ;)). Do you drink beer often? Cut it! FYI, allopurinol “treats” overproduction but it’s the first line of therapy even if you excretion is not optimal
  • Underexcretion: uric acid is eliminated through urine. How can you excrete less UA? first issue is obviously kidney disease (you are not suffering it, I took a look at your labs, your eGFR looks well), but even in a setting where kidney function is OK, are you drinking enough water? Less water = less excretion + more reabsorption of virtually everything through the tubule. I assume you are not taking meds such as diuretics (for medical condition or fitness purposes, they are a cause of high levels of uric acid as well due to dehydration = less urination ? less excretion). I would increase water intake if u deem it not enough.
    Check it in some months, cut red meat and escessive alcohol, increase water if not drinking enough and if not going down, start allopurinol at your physician’s discretion :wink:

Thanks. Yea just want to get it in check . A few people I know have it and doesn’t look enjoyable. And these people are all a lot older then men . Plus they also drink heaps and eat like shit.

Is there a way to work out which of the two could be happening…?

Either Overproduction or Underexcretion.

For the overproduction I was eating a lot of red meat. Mince 2-4times a week. Plus Steaks on weekend. Not sure if this was the cause. But I have cut this back.

I haven’t drank beers in the last 6months.

I did start trt about 16 weeks ago. Some things I have read said this can cause uric acid levels to go up. But some people tell me this isn’t true.

I only recently heard about underexecretion.

I read it can help to drink small amounts of bicarb soda to help excretion of the uric acid. Haven’t tried this yet.
Said it to do it for flare ups.

Also trying to alkaline the body more. One thing I saw said acidic body makes it more likely to get.

Does allopurinol have any long or short term side effects to worrie about

I do. Talk with your doc. Gout is painful. I do not drink, and do not eat like :poop:. Usually. Although I have an allopurinol (sp) prescription I do not take it. I have neuropathy. Honestly, I am not sure when I have gout or breakthrough pain from the neuropathy. Good Luck!
