Test Results/DHT/HCG

I’ve been on defy medical’s testosterone protocol for two months now and my results are as follows:
Total T: 939 ng/dl
Free T: 232 pg/ml
Dht: 44 ng/dl
E2: 29 pg/ml

Initial protocol:

Test cyp 150 twice/week *Defy told me to take 400 iu of hcg, but didn’t take it until 7th/8th week.
Anastrozole

My concern is that my dht levels are still low despite an elevation in my testosterone levels. The ratio of T:Dht should be anywhere between 5-15 - mine is at 21. My current doc, Dr. W (not Defy), interpreted a blood test result from a precursor of dht, 3a androstanediol glucuronide (since my lab provider did not test for 5a reductase deficiency). My 3a level was 838 ng/dl. According to Dr. W, if I had 5a deficiency, then my 3a would be low as well. Anyone here have an opinion about this? Also, Dr. W advised not to take hcg while on my test cyp regimen. For the first 5-6 weeks, I didn’t take hcg. Now, I have changed my mind and for the past 2 weeks have taken hcg at 400 iu Mondays/Thursdays on injection days (as originally prescribed by Defy). I have noticed a bit of atrophy and am wanting to know if 400 iu twice a week is enough to bring my balls back to their former size. Advice would be greatly appreciated!

You doctor is just not very well up to date on current TRT protocols. It doesn’t seem like your regular doctor cares about testicular atrophy or whether or not fertility is at risk! I wouldn’t want this person in charge of my health.

Since when is staying fertile while on TRT is a bad idea?

TT looks good, but I can’t really comment of FT levels, no way to tell were levels stand without reference ranges.

Defy doctors are usually ahead of most sick care doctors in TRT knowledge because Defy prescribes TRT on a daily basis whereas doctors in sick care do other things and only TRT on occasion.

Free t is on a range from 35 - 155 pg/ml

Dht is on a range from 16 - 79 ng/dl, but I think defy sets the range at 30 - 90 ng/dl, so I don’t know.

400 IU’s 2x a week should bring things back ball size wise

It is simpler to start with only T, see how you feel once stabilized, then decide whether to add HCG or not. Though, starting it from the beginning is less risky in terms of fertility.

What about my dht levels?

How is libido? If good, I wouldn’t worry too much.

Libido is good some days others not so good, but there are other factors involved that affect that.
