Psychological issues are not so easily separated from organic causes that are best treated, in part, by medication. But I do agree there is an over-medication in the field of psychology. Particularly things like ADHD-much of that has to do with abuse, as adderol, ritalin, etc… really do help with concentration and foucs even for those in the normal range. Some of the over-medication also has to do with practical issues. Drugs cause money. But therapy is more epensive. And time-consuming. So people are inclined to pop a pill. That’s the unfortunate reality. There are many good psychologists and psychiatrists. There are also a lot of bad ones. It has its problems just like any field.
This belief of mine is reinforced by watching the events around a close relative of mine. Nice girl went in with a minor problem, and suddenly all hell broke lose, and this nice girl turned into a total nightmare. The parents took charge, pulled her out, and she reverted to that nice person again. Years later a very bright and becoming successful person. And interestingly with that same initial problem that was supposed to be dealt with in the first place. (Little obsessive/compulsive behavior.)

210 pounds for a goal? Dude, I haven’t been 210 since I was like 14. And I don’t care what you say it can’t be done in a year without surgery. That is 20 pounds a month for a FULL year. Surgery & cancer maybe. Diet & exersize?
Ok here is what you need to do to get to a normal weight.
Get off the phentermine. It’s crap and if your doctor is giving it to you then get a new doctor. At your weight you are a heart attack waiting to happen.
For the love of god stay the hell out of Burger King and of the other fast food places. I managed a BK for 7 years while finishing high school and all through college. You don’t even want to know what is really in that food. Trust me it ain’t good!
Get a blood panel done that checks your cholestrol, triglycerides, prostate, DHEA, testosterone, estrodiol, thyroid etc. I’m betting that at your weight your body is pumping out some massive amounts of aromatase enzyme which is turning your tesosterone to estrogen. If your test is low and estrogen is high your gonna have a hell of a time losing weight regardless of how much diet and exercise you do. You may want to go so far as having a stress test done for the ske of your heart.
As for exercise, start with cardio on the treadmill or swimming at least for a few weeks. Then when you move into the weights, start with a circuit program that uses lighter weights and more reps. The book New Rules of Lifting by Alwyn Cosgrove and Lou Schuler has a fantastic 3 phase fat loss routine utilizing the basic movements.
I know you said you hate supplements, but you should at least take a good multi-vitamin, Omega-3 fish oil & utilize a good protein powder/meal replacement (Metabolic Drive, MYOPLEX, MET-RX)
Take a picture of yourself in just a pair of boxer shorts. Hang it on the refrigerator for all to see. Do you like how it looks? No! Good, there is your motivation to lose weight.
Get rid of the name Fat Tony, Big Tony whatever.
Maybe try Anthony, it even sounds slimmer.
