If I get anterior shoulder pain (front of humeral head seems to translate forward and pinch something) when doing shoulder high INTERNAL rotations but no pain with external rotations, what might be the problem, and how should one fix that? [/quote]
Two things could be happening here.
Your subscap isn’t strong enough to be performing them and the humerus is gliding forward because it is weak. Really try to retract the scap the entire time you perform them and keep the weight light and form strict.
The second and most likely problem is that your external rotators are tight, so every time you place the shoulder in a position of extension and try to internally rotate you get the “glide forward” again.
Perform the sleeper stretch or cross body shoulder adduction stretch once per day 3 sets of 15 seconds for a few weeks. You can even perform the stretches for the external rotators then go ahead and try to perform the exercise again. You will probably find that the pain will go away.
