It’s like telling a joke. If you don’t get the point then it’s not worth explaining.
OK-here goes. This woman is sick. Someone could have helped her (possibly) at any time prior to her decision. Someone could have cared and she could have learned how to act against her sickness or instinct or whatever drove her to this action.
