Delayed Puberty?

Hello, I am 23 years old and I feel as though I have not gone through puberty completely. Currently I am 6’1 245. I’ve always been tall, but since high school I’ve always felt underdeveloped compared to my peers. Now, that is more evident. I constantly get mistaken for being 16 and the other day someone said I look like a big 4 year old. Downstairs I feel as though im the same size as when I was 7. Despite lifting heavy, I still can’t put on muscle. All of these things lead me to believe that I haven’t gone through puberty fully. Currently, I am on trt taking 75 mg of 100 mg test cyp. I have been on this dose for about 7 weeks now and still have seen no changes physically. I am getting bloods and talking to my doctor about this next week, but I was wondering if anyone else has experienced what I am going through and at what dose was puberty induced and gave the most physical change? Also, Im aware that this is a marathon and not a race, but 7 weeks of trt and consist weight lifting, I feel as though I should have seen some sort of change physically. Only change is people tell me I appear to be slimmer.

I can tell you that you will need to at least double your dose to get the physical changes you are seeking. Did you get any testing prior to starting testosterone?

That’s what I was thinking. I honestly was thinking about talking to my doctor about getting on around 150-200 mg / week. I did get labs done will post. One second.

I believe last time I measured it was around 5-5.5 maybe. But not gonna lie, it looks like an infants tbh. Even just the girth and head development aren’t there. Tmi I know, but yeah.

These aren’t the most up to date labs as I cant find those right now, but I know they were even lower than these. I know for a fact that prior to my first injection of test, my testosterone levels were at 200/1000.

I’d go to 150mg. See if you can get E2 tested next time.

Will do, thanks. I seem to be fine with 75 mg once a week, I assume because it’s a low dose that’s why im not experiencing side effects. If I do get the increase, should I stick with weekly or change to more frequent injections?

Wow okay so after doing some research signs point to me having Klinefelter syndrome.

This is average size, not everyone is going to be satisfied with the genes passed on by their parents.

Your protocol is inappropriate, SHBG is on the lower end so you are binding less testosterone and the majority is bioavailable and you are probably clearing out testosterone quickly therefore weekly dosing is not ideal.

Delayed puberty can be cause of low growth hormone levels, low testosterone as well. If you missed that growth spirit at the appropriate time, you get no second chances.

I take 200mg a week with no side effects. My SHBG was 21 when tested two weeks ago. Most guys have no side effects, maybe 5% do. Most, 90%, of the guys on TRT take 150-200mg injections once a week.

Up your dose. Take it once a week. If you find you consistently feel run down towards the end of your week, it’s a trend, and cannot determine a reasonable cause or explanation for always feeling tired on day five, or whatever, then increase to twice a week. You can always try more frequent dosing later. For now, get the dose up, take your injection, and enjoy your life. Walking around all the time thinking about your nipples, whether or not you a retaining fluid, wondering if you are off, not feeling “stable”, etc. is no way to live. Guys obsessed with side effects usually find them.

Im aware that’s the average size, but I think I didn’t develop all the way. After doing some research Im almost 98% certain that I have Klinefelter syndrome. The way my body is shaped, where I store fat, the gynecomastia since I was 13, the prepubescent facial structure, the small testes, the lack of confidence, the fact im significantly taller than everyone in my family, it all points to Klinefelter.

I’ve been trying to find more people who have the syndrome and went on trt to get an understanding of what could happen if I get the right dosage. I was able to find one person with the exact same problems as me. He said after about 4-5 weeks of taking 200mg his body changed significantly and it was as if he finally went through puberty at 22 years old. Im hoping the same for me. I know you say if I missed the growth spurt then it’s too late, but maybe not if I never had enough testosterone to even go through puberty in the first place.

Hopefully at a higher dose, I can finally be a normal functioning 23 year old man. I hate constantly getting mistaken for 17. I hate how feminine my body composition is.

But didnt you get a proper check up by your physician before starting TRT?
I think its absolutely the right thing to do, but anyway an MRI (in case your prolactin is high which could suggest a treatable pituitary tumor) and/or a genetical testing for Kallmans or Klinefelter (which is likely the cause, i agree) would be indicated at your age.

I also agree with others that you want to have higher T values in your case. Something between 600 to 1000 can be considered physiologic.

Ask for a bone density measurement as you will quite likely have osteoporosis. It will however in most cases improve during TRT.

Do you have a reduced sense of smell?

Good luck - it will take some months to grow wings but then you will be ready to fly!

Do not ignore this. You need to go see a good endocrinologist. Maybe a pediatric one??solution may be same TRT. But if diagnosed earlier you would have developed better. Now your growth plates am sure are all closed.

Well I first discovered I was low test from my urologist but he was scared to treat me so now Im at an anti aging clinic. I got all proper bloodwork, but in terms of a physical check up, I didn’t get one. But wow the osteoporosis would explain my chronic lower back pain when I was younger. I always thought it was just from playing sports. I should have gotten it checked out but I never really told my parents.

So would I still develop? I dont care about growing anymore in terms of height, but I’d love if everything else matured lol. I found someone on Reddit who was very similar to me, and he said when he started trt he finally went through puberty.
