25 y/o. Is my LH/FSH Low? Low T, Low SHBG, High TSH, Insulin and Leptin Resistance

Hi, guys. I am about to talk with the doc and try TRT, but the doctors in my country are not some you can count on. Please, help.

My current condition:
I am 1.92cm, 135kg with central obesity, mostly subcutaneous fat (feel like having excessive skin, but with lots of fat on it). I have brain fog, lack of concentration, very low energy, constant ringing in ears (got used to it), unable to lose weight with professional trainers, fitnes, diets that worked before and anything else. Once dropped to 122 kg and got stuck for 3 months without negative diet change or excersise change. My sex drive is lower than before, but still very good.

How long I have this condition?
I have this condition for the last 3-4 years and before that i was 1.92, 93 kg without any of this problems. Not sure how long i had the ringing, but it is possible that i have it from before.

My Background:
Normal, tall and strong kid BUT when I was 10 y/o something happened and I started to gain weight and smoking (East Europe… what can I say) and 1 year after that I stop smoking but the weight remain until pubery when it got fixed. Only my love handles were fatty.

From 14 to 22 y/o (had no chest hair but my father said he didn’t have either at 22 and his T is higher than mine even now). My weight was stable. From 82 kg 1.86 in early puberty to 1.92 93 at 17 with lots of sport (judo, fitness, basketball, soccer…) and the problems started at the age of 21… at first only energy problems and at first I though its protein and for the first time ever I tried supplements (only protein). Well… helped me for a month, but after that… the lack of energy made me quit trainings and tried to eat less (back then I was eating A LOT but got normal 95-100kg body, not yet fat). I lost muscle mass and started to gain weight. Then tried to do sport again and every time I did sport… my weight jumped higher and I was unable to get it back to normal. My last 6 months of traning ended earlier this year… and i gained 13 kg while trying to lose weight even with professional trainer. My diet was fine. So… I am stuck now and afraid to do sport and only tied couple diets. Only the low fat diet works, but my energy goes below 0… and even if i sleep 12 hours daily… i feel tired.

My current health:
I am giving you pictures of my lab results and i hope you can give me some advice, because our docs are trying to label me as normal Hashimoto case and I believe there is something else, since my Hypothyroidism is only Subclinical and not yet that developed to do such damage to me.Results in the comments:

Please urgently do an MRI to get checked for a pituitary tumour. Your symptoms are very similar to mine, and those that my father had, and both times it was a slow growing pituitary tumour which disrupted the pituitary gland’s work. The MRI takes 30 minutes, but can save you from blindness and death, kinda worth it :slight_smile: … if no pituitary tumour, then get your SHBG under control, I did that with 6mg Boron and 2000iu vitamin D daily every morning. Keep your iron in check, and eat only vegetables, and meat, and cheese, no fruit, no sugar. I’m not saying that the above will solve all problems, but that’s where I would start of I were in your situation. Good luck bro, and keep us posted.

Note: I have other stuff tested, but I doubt you will see anything different. My prolactin was normal, cortisol bit low, but still fine. I can give you more later. I did proper insulin test 3-4 times. All the same. Can’t get my blood sugger low enough. My sugar is around 5.6-6.0. (max 6.3).

I don’t smoke, drink or do drugs.

Thank you very much for the advice. That’s what I though, but docs told me because I have normal LH/FSH… tumor/adenoma is not an option. I will do an MRI even on my own just to be sure. Also… the discribed diet from you helps me the most. When I was in the gym, this diet was my worst, but all better diets are now worse, and this one seems to be good for me so far. Thanks.

About the SHGB… currently I have sex life… even pretty good actually which is not common case from what I see on the internet, but I bet this is because of my high SHBG. That is what keeps my Willy strong… don’t wanna start treatment from SHBG :frowning:

Your SHBG is low, not high, 11.83 with a ranges 18.30 - 54.10. TPO antibodies is very high, thyroid is under immune system attack and struggling. Testosterone is very low, that of a 90 year old man. I’m unable to read some of your labs, if LH is normal and testosterone is low that testicles failure.

You are right. Sorry about that. I still don’t get how SHBG works, but anyway. Thank you for the comment. I went to the best doc in my country for a physical exam twice and he said it looks OK. He blame my Thyroid for the low T. My question for you is… is there any possibility that my testicles are fine? I Don’t really feel like there is something wrong with them. Keep in mind that my sex life is fine. My sperm is at 2% though, yet I feel fine. They blamed the Testosterone about the sperm. I am currently at the hospital and they said there is no way I have pitulary problem, but still… give me your opinion, please.

EDIT: My LH/FSH are normal they said. Didn’t see the results yet. 1 year ago seen them low, and on the next test they were normal.

@jakob613 I just came back from the hospital and I have my MRI results. Looks like my pituitary is completely normal. So… my question is: If my testicles are fine and my pituitary gland is also fine… and since i have never took any testosterone or any supplement… then what possibly can be the problem with my testosterone? Keep in mind… my HDL and vitamin D are extremely low. May this be the answer?

Most guys never find the cause of their low testosterone, day in and day out we see no problems with the pituitary gland or the testicles. I’m sure in the years to come we find the endocrine system disrupting chemicals.

@systemlord Sadly. What would you do if you were me? Is TRT a good option? It may fix my insulin resistance, concentration and weight maybe… Since I don’t really have sex problems… that’s all I ask for… But is it the right decision?

Fix the thyroid and see how things go. It’s common for hypothyroid patients to experience scatterbrain and weight problems. It doesn’t sound like the doctor is going to give you TRT without first addressing the thyroid.

@systemlord Like I said… my hormones are normal. T3 is right in the middle. T4 is lower, but still normal. There is nothing I need to fix i guess? The only thing to fix here is the antibodies I think, but if I didn’t miss something, no one knows why are they fucked. Can my IR be responsible for the antibodies? Who knows. Or the trt? Since I have normal T3… i don’t know how can thyroid be the problem here… Dunno what to think or do. 3 years later… one day I feel like I finally know what’s happening and the other - I feel dumb and lost in all those possibilities…

Not to mention that… when I take T4 - my TSH jumps higher… and T3 is still the same.

Your TSH is 8.59, TSH becomes elevated when the pituitary gland detects a problem.

@systemlord So lets see if I understand the situation… TPO is basicly building T4. Anti-TPO is basicly against the T4 production. Since T4 has no other use than becoming T3… there are only 2 options - either my body simply hates TPO for some retarded reason I can’t find or cure (bad, but nothing I can do about it, and giving more T4 will probably only boost TPO production and increase the immune reaction resulting in nothing good, especially when I don’t really need T4 at the moment, because I have enough T3) or for some fucking reason, the thyroid gland works too well and feels like I have too much T4 produced, but cannot say it to the brain and simply produce anti-TPO to take a break?!

I don’t know what is the reason or what’s going on but I don’t know what can I do about the thyroid since the T3 are fine. I have a friend with normal hormones who was born with only half thyroid. That’s where I got the idea about the second possibility. Basicly the brain should be able to check for the T3… so why is TSH high? There are other possibilities, but I DONT KNOW THEM… and trust me… most of the docs in my country don’t know either. If you know anything else than T4 ot T3 pills that may help my TSH and anti-TPO - say it.

Thyroid medicine will not affect the TPO antibodies, the antibodies will continue attacking the thyroid until you find the cause or find a way to modulate the autoimmune disease. This is where a skilled thyroid doctor is needed, if the doctor doesn’t know what to do he will simply brush it off and say not to worry about it.

Well… Dunno what to do. They told me to check my testicles again but this time not only physical. Best doc in my country said we must elevate the T… i guess with TRT, but I am too scared to fuck it up even more. My hope is that someone here my have had the same problem.

I have had 3 or 4 times already. It is “damaged” , but normal size.

With a TSH that you have over 8 needs to be treated some how. The damage will get worse each month and year.
How you get it treated, you need an Endocrinologist who understands thyroids.

I know, but I can’t find good doc in my country. That’s why i’m here. Everyone so far told me to take T4. They don’t even test my T3 & T4… i did test them by myself. So far I paid so much money to test things on my own… And I don’t care about the money but the health.
