What’s More Important TT or FT?

Been considering TRT as an option for a couple years now. I’ve always been natural my whole life, but over the past couple years I’ve noticed an extreme amount increase in daily fatigue (need to take naps daily) and extremely high levels of anxiety (I refuse to take any meds for this and alternatively try to meditate, and perform breathing exercises). Before going through Defy, I reached out to my primary care doc and basically had to beg him to give me a testosterone test - the only test that was performed was total testosterone and my numbers came back at 412 TT. He told me I was fine and the conversation stopped.

I finally decided to get my blood work from Defy to evaluate if I am a candidate for TRT.

Here are my levels and stats:

Age: 30
Height: 6’5”
Weight: 238 lbs

Triglycerides: 167 Mgl/dl 0-149
TT: 512 ng/dl scale: 264-916
FT: 7.5 pg/dl scale: 8.7-25.1
DHEA: 292 ug/dl
Estriol: 11.9 pg/ml scale: 8-35

It looks like my free test is really low, but my total seems within range which is a little confusing. Not super inclined to just jump on TRT for no reason, but def looking to solve my daily issue that I have experience. Excluding high levels of fatigue and anxiety, I fortunately do not suffer from low libido.

Any help or recommendations would be greatly appreciated.

At your age, I would recommend covering the nutritional side, before seeking pharmaceutical intervention. Get your nutrition spot on.

Agree with @RT_Nomad. TRT is not a lifestyle it’s a treatment for hypogonadism. Now I know Defy tested more than that and they are also hesitant to treat <40. I would exhaust all options before going to TRY at 30 which includes whatever lifestyle changes you can make and testing more than just once.

how much are you sleeping and what does your diet look like?
How often do you engage in resistance training?

To answer your question, FT is ultimately what impacts how you feel, but doctors will try to keep both in range.

Sleep is bad. Always has been. Take melatonin /ZMA which helps a little, but overall my sleep is horrible no matter what I try. My diet is pretty solid, and train min 4 days a week.

Curious if minimal weekly dosage would help increase my FT, and also improve sleep?

have you had any sleep studies done?

You didn’t comment on training or diet, which tells me both of these need work.

TRT isn’t going to help you sleep any better. Even a minimal dose will still shut your balls down.

I’d get another round of tests just to confirm the low FT reading.
If still that low, yes, i’d give it a try.

The fact you have libido NOW is a good indication that if you go on TRT and it vanishes, it could likely be found.
