Trying to Diagnose Thyroid Problem or Hypogonadism (23 Y/O)

Hey guys I have had a long issue with feeling tired a lot, lack of motivation, bad anxiety, low libido, getting hot flushes ect. My strength in the gym has actually been okay but I do get fatigued quite easily. I have attached my blood work for thyroid function, t levels and fsh/lh. I have got results months prior to this and they were the same. I have had other blood work done as well if you’se wish to see it if it will help with deciding if it is my thyroid at fault or not. I am 23 years old, have a clean diet and have been training for 4+ years. The doctors, endos and urologists I have seen all seem to not be quite sure what is giving results like this. And is it getting quite expensive going to and from specialists and still not getting the issue resolved.

It would be nice to see T4, Free T4 is way too high pointing towards hyperthyroidism, thyroid is overactive. Testosterone production is the opposite. Good thyroid doctors are very hard to come by in the UK, suggest doing an advice only consultant here. This way at least you could have some answers and a plan.

When you decide to start TRT the protocol will need to be either an EOD injection schedule or an every day schedule do to low SHBG. Your testosterone is down from 7.3 to 4.3nmol/L, SHBG is + 2 points. Free T is down from 212 to 114 pmol/L.

Any medications, iodine supplementation?

Do you think an over reactive thyroid could yield such low t levels? Or the only treatment for that would be trt? As I am trying to avoid trt as much as possible as I am still young. Also with the trt if you use a long ester would it still be requited to use it EOD?

I did try iodine supplementation for a while but with no positive results. And no medication, no supplements used. I do have high albumin levels aswell which the t could be binding to

Higher albumin levels may be caused by acute infections, burns, and stress from surgery or a heart attack.

How high is albumin?

Albumin 51g/L and cortisol 440nmol/L

That’s very high, normal range is 3.5 - 5.5 g/dL. That high albumin is forcing T4 bound hormone to convert into free hormones too soon or rapidly which is why your Free T4 is so high. It’s similar how low SHBG creates more free testosterone.

You need to figure out what’s causing your albumin to be so high.

Do you have ranges for cortisol? US measurements are different.

cortisol ranges are 85 - 460 nmol/L and I had 440nmol/L which is very high, which i believe is a result of my anxiety, depression ect possibly caused by really low T. Albumin ranges 36-49 g/L (3.9 - 4.9 g/dL).

I just find the whole situation very weird because this is probably the strongest I have ever been, the heaviest also. But my T levels have never been so low. Physically I don’t look like I have low T levels, but blood work indicates otherwise on all multiple occasions.

I believe the high cortisol could be caused by low T and thyroid issues. Fix the testosterone and thyroid problems and cortisol should come down.

I wish I knew how to fix the low t and thyroid issues if I knew how or if only 1 of them is at fault and causing the other to be effected by it.
