TRT Labs Question

39/M 6"4’ 225lbs around 15% body fat on 150mg on test cyp once per week for the past eight years. I’d say over the past year I haven’t felt great. Thoughts on these labs? These are peak numbers, two days after my shot.

How do I get free test down? I’ve tried splitting shots into twice per week but typically end up feeling depressed. Longest I’ve given that protocol was two months. Give it longer?

Total T 1043ng/dl
Free T 265 pg/ml
Shbg 19 nmol/dl
E2 ultrasensitive 41pg/ml

Go back to weekly dosing since it worked for so long, just lower your dosage, 150->130mg.

Maybe it’s not the TRT protocol that’s casuing your problems.

Two months is about the time it takes for your body to adjust, adapt.

I’m on Jatenzo reaching steady states in 7 days and still takes two months for my body to work things out.

Your E2 is a tad high. High estrogen in men can lead to depression. And remember, “normal” lab values in general probably apply to maybe 95% of the population. That leaves an awful lot of outliers. 41 pg/ml might be significantly high for you. Have you tried subcutaneous injections? Less aromatization. Maybe drop dosage a bit as systemlord suggested.

They are fine.

Decrease testosterone.

I think I would go back to what was working. Then, reassess and see what your doctor thinks.

Thanks for the reply. I guess my concern is that free T is double the normal range and total is within range. Dropping 30-40 mg isn’t going to get free into range. E2 will likely look better but free T keeps sky rocketing even though SHBG isn’t that bad.

I agree. E2 is a little higher than I would like. I guess my bigger concern is that free T number. It’s wayyy over the top of the normal range while total is in range. My SHBG isn’t terrible, I don’t think…

Even my trough numbers show free t above normal range while total is around 500.

FT is just fine

I wouldn’t worry about the high free T. In itself, higher than “normal” levels of T aren’t inherently dangerous or problematical; it’s the occasional side effects of high T that you have to watch out for (high hematocrit, hemoglobin, E2) and you’re getting a handle on that.

Your free testosterone is fine. However, I gather that you think it is a problem for you as you asked how to lower it. If your solution is lowering free testosterone, and you desire to have it in range (I assume in range at trough because it will be above the range 24 hours post injection), then the answer is simple.
