So I’m a 40 year old male who has been on TRT for around 6 months. Currently my dosage is at 65 mg of test cyp every third day. I am also taking 25mg of Aromasin every day. My latest Labs (which are nearly the same as the labs I had a month ago show:
Testosterone, Free and Total
Testosterone, Serum 1032 ng/dl 348 - 1197
Free Testosterone (Direct) 40.6 pg/mL 6.8 - 21.5 (High)
Estradiol 24.7 pg/mL 7.6 - 42.6
My first question is, what are the issues with having such a high Free Testosterone level? Two, what can I do to bring down my free T (other than dropping my dosage as I like having my T levels somewhat high.) I appreciate any ideas you guys have.
You don’t know that for sure. The test for free T is a function of total testosterone and SHBG. They use an equation using the two values because T binds strongly with SHBG. The issue is that it ALSO binds too other proteins in your blood like albumin and others. So when you see a result like that you have to ask if it makes sense and whether it’s valid. It makes sense with your T at 1000 which you don’t want to lower. But free T may not be as high as the test indicates.

Hmm, I would have thought that having T at a 1000 would have just put my Direct T up at 21.6 and not double that. As an FYI my Labs last month had me at TT 912 and DT at 40 as well. Regardless, thank you for the input
I will say that having T levels this high has only improved my mood and being on TRT has made me a happier person (I used to be a really grumpy MF’er for no good reason with a quick fire temper and now, I’m pretty mellow.)

Worried that my Doc will look at my results and start mucking around with my dosage. I’ve been getting my own labs done to make sure I’m where I need to be.
To answer your question though, I’m not having issues. I feel great and was just worried about the DT results of the Test and what having high DT really means (if it means anything)
Op you have low shgb so your testosterone isn’t as bounded up. Your results look great. If the doctor let’s you self inject you can always drop your test dosage to half and the ai to half.
The relationship of being on the high side of a range does not mean that you’ll be on the correlating range of another. Way too much emphasis and advice here is given based on a number in relation to a range. It’s so nearsighted. There’s always a bigger picture.
The Doc knows enough not to use the Free-t as a valid indicator to reduce dose. Combined with the total T of >1000 he might be inclined to. But, he’s also going to assess your signs and symptoms as well as all the other lab results.
I feel best with Free T towards the top of the range. My mood, energy, libido and sexual performance were hit or miss if I was above that by much. My goal is always to take the least amount of drug for the desired effect, especially when it has to be processed by the liver. Who knows what Aromasin effects long term. Why not try to take less of it by reducing your T dose to just what you need to feel good?
