TRT 1 Year Update

It’s amazing he doesn’t support the use. Yet the body creates it for a reason and converts preg to DHEA naturally: its like saying the body doesn’t know what it needs.

Not sold yet on keeping dhea on the high end. We are on exogenous testosterone. Dhea maybe is lower cause we don’t need it to convert to testosterone.

I had 2 endocrinologist and 1 urologist tell me that. And the endo on here physiolojac

Am leaning towards only supplementing if low end.
Plus I want to know if my dhea s crashes cause that indicates adrenal issues and affects more than dhea

I think it’s a smart idea to keep it at levels that are healthy and not letting it go too low.

So I decided to start metformin. I used this Calculator.

I also hopefully temporarily stopped daily cialis. I want to be sure it’s not causing my heart paps. Last I took was last Thursday. Will wait until this Thursday and access.

Also just received last 2 remaining labs. Igf and am cortisol.

My igf was slightly elevated a few times and is again. I also would think my am cortisol should be higher. Perhaps start of adrenal fatigue. Idk. But I feel good.

So went to see Endo today.
She asked to see me after lab work.

Being I have slightly elevated calcium levels and igf1 she is checking to make sure I don’t have Acromegaly. She doubts i have it.

Funny thing is acromegaly would explain my surprise cardiomyopathy. And it would indicate a tumor near pituitary. Which can explain secondary hypogonadism.
I did have a mri 2 years ago without contrast. That was negative.

I will continue metformin.

So to check for Acromegaly it is a glucose tolerance test that checks Growth hormone, glucose , insulin during the 3 hour test.

I believe my igf1 is slightly elevated cause of TRT. My igf was 246 (76-216)
Calcium 10.3 (8.7-10.2)

That’s interesting. I think just about everyone on trt gets into the high or higher than normal lab ranges. Shouldn’t we be checking prolactin?

Found this on google. I remembered a young guy who had high prolactin and the doc suggested having this checked before mri.

One common cause of hyperprolactinemia is a growth or tumor on the pituitary gland called a prolactinoma . The tumor produces high levels of prolactin . These tumors can be large or small and are usually benign, meaning they are not cancerous. Large tumors can also cause headaches, vision problems, or both.

I knew someone with this, it is a very definite look. If you still have the same shoe size you did in high school, you don’t have it.
Any labs on Vit D or PTH?

Last vit d was in 30s. In range.
Pth last checked in 2015 was fine

Pth is being Checked Thursday when I do glucose test. Along with am cortisol and acth. And metabolic panel again.

Prolactin has been high normal or just slightly above range.

The only thing that grows faster than b4 TRT are nails and hair.
I did not notice and bones in face showing and shoe size I think is the same.

The calcium should get checked at the same time as PTH, or PTH doesn’t tell you much. They live in balance, and PTH has a half life of 5 minutes (technically it’s supposed to be collected and stored in a chilled tube - but they usually don’t), so you need to know what they are in the same “snapshot”. They should not both be high, and 10.3 calcium is high for anyone older than 20.

The metabolic panel includes calcium btw.

I had a lab 2 weeks before that on 10/3 and it was 9.7. And 9.9 in May. I feel better checking back. Also igf in 180s in May. I was injecting less than. So I think am getting a good increase with TRT in igf which body builders would like.
Right? @hardartery good sign that it may be lab variability I hope.

So if I had a parathyroid tumor I think it would be high then also.

For a tumour, yes. It would be generating retarded PTH without letup. For me though, no. I had hyperplasia, so my levels went up and down depending on my Vitamin D. It is unlikely and really rare, and the doctors don’t want to treat it because it is frequently classified as “mild”, because they know less about it than TRT.

So I don’t have Acromegaly.
Hgh was suppressed in the presence of higher glucose.

Also my 3 hour glucose tests- glucose readings all below 100.

Dr also checked insulin levels during 3 hour. Was within range but said could be better. So I do have mild insulin resistance. Am already on metformin.

Calcium back under 10. PTH normal.
the probiotics I take in am has calcium in it. And I took it the day of my last labs. I think that’s why it was over 10.

So my igf is slightly elevated and she said no worries About that.

I stopped daily night time magnesium glycinate a few weeks ago and it seemed to stop my heart paps I got at night laying down before sleeping. Accompanied with that was a few times a week I would get the gasping for air right before sleep (right when you are in and out when just starting to sleeping). That stopped as well.
Go figure.

I also am doing 10mg Cialis eod instead of 5 ED and I think it helped with daytime heart paps.

My last echo with new meds (beta blockers and an ARB) has strengthened my heart. Me ejection fraction is up to 55% from 45.

I feel good. I just took a bunch of labs to see if the readdition of metformin about 6 weeks ago is affecting my numbers.

I inject 110-115 mg a week divided in 2 subq injections.
I also added 500mg resvetrol 2x a week about a few weeks ago. I started ED but just recently switched to 2x a week. I read it can act as an AI.

Will see what labs results are next week. I do not want to lower my e2. When I do I get joint pain esp lower back pain.

Vitb12? Is that considered low?

VitD dude. Id fix that.

Was this fasted? Triglycerides are high. Cant remember if we’ve discussed that before.

It was fasted. Triglycerides like that for years and years. Was even higher when younger.
Diet changes don’t help.

Am ok with it since I have zero plaque score when I did cardiac ct. Dr really doesn’t want to treat unless it’s higher.

I will most likely start fish oil which suppose to help.

What do you think of b12?

Mid range but your Vit D is low.

I have an issue with belly water retention. There are days that I weight 188 pounds then a couple days later 183.
Belly feels bloated , you know that helping that you are pregnant and look it to but you are not fat.

So I see my options are as follows

  1. Stop the dhea (I was low normal)
  2. Reduce testosterone (I am a bit above mid range on free t)
  3. Stop the metformin (I don’t think it’s this)
  4. Ask my cardiologist to add a low dose diuretic to my beta blocker or ARB.

What do you think?
