Hoping to find some advice or information,
I recently asked my Dr to run a testosterone blood test as I had just turned 40 was curious and without going into all the details I have been feeling some what off for the last year.
The results came back as follows:
Total test 2,13 ng/ml
SHGB 14,35 nmol/l
Free androgen index test/shgb 51,5
My concern I raised with him was that my total Test appears to be suboptimal for my age but his response was the androgen index was fine so the ratio was good and therefore he had no concerns and would not take that matter further.
I am just wondering what peoples thoughts are as even if the ratio between Test and SHGB appears good surely if the total test is so low I just simply don’t have enough test available ?
Any thoughts or further information would be greatly appreciated

Could you update this with the ranges on your results sheet? And are those commas, supposed to be decimals?
Sure thing , here it is
Test total 2.13 ng/ml - range 2.49 - 8.36
SHGB 14.35 nmol/l - range 18.30 - 54.10
Free androgen index 51.5 index > 35.0
I assume the commas are decimal points, the test was done in Germany and I think commas are used instead
Thanks. So how do you feel? Regardless of lab tests. We have all found that Dr’s rely way too much on lab results and whether they fall in the normal range rather than how you actually feel?
I get that your free androgen index is good, but that is calculated on a very low test level. Seems misleading. A good ratio of a very low amount is still low, right?
Either way, I think you should look for a TRT Dr to help you. Unless in Germany TRT docs are rare.
Doctors aren’t supposed to go by androgen index, the guidelines are specific about Total T and Free T being considered and one are at the levels of a 100 year old man.
Clearly this doctor has no business diagnosing or treating a man or a woman with hormone deficiencies. This is the very first time I’ve ever heard of a doctor going solely on androgen index.
Any TRT protocol that doesn’t focus on injections every other day or everyday will likely provide poor results do to low SHBG.
