Your levels are not low, they are dreadfully low. These are disease state levels, there’s no doubt about it. By US measurements your TT is 250 ng/dL.

It usually doesn’t really matter the cause to be honest… the treatment is the same either way. TRT.

Rather than starting trt I would like to see how it could be possibly fixed.

Outside of a known trauma (car accident, horse kicking you) the cause usually remains unknown. Probably unknowable right now no matter where you are.

My point is the cause is very rarely ever determined, and even when it is, there usually isn’t a way to “fix it” in the way you mean.

The usual recommendation for low SHBG is frequent injections, daily if possible

Last resort, only if there are no other options.

Androgel usually isn’t strong enough. HOWEVER, there are testosterone creams now that are much stronger, but you have to get them made at a compounding pharmacy. I don’t know how this works or if it’s even an option outside the US

The free and bioavailable on my tests seem to be mid range is that not ok?

I try to exercise 2 times a week.
I’m 5ft 9 153lbs I dont know my body fat I’m not overweight very very small belly.
