No libido / Energy on TRT [Low SHBG, High E2, High PRL) Tried so much

I’ve been trying to feel good for 2 years. During this time I’ve tried Cyp + Enanthate (EOD + E3.5D Propionate (ED), at varying doses (800ng/dL - 2000ng/dL)

Nothing has ever made me feel how T should feel. I have no energy to get up in the morning, no libido, no morning wood.

The first time I ever got on 2 years ago (Cyp) I felt great for the first couple weeks, felt libido creep in, (this still happens when changing dose substantially) but then everything drops off.

Weird things to note:

Occasionally after heavy drinking (the morning after) I feel great, how I should feel on T except the effect on cognition via the hangover behind it. I have never been able to explain this as my E2 is ALWAYS high anyway - explained below:

AI: E2 in my bloods always seems high. However, taking an AI (Anastrozole) gives me low e2 symptoms before I feel better, always, no matter the dosage - it’s on the E2 rise back up I have 1 day (Max) of feeling good (not optimal) again, then back to feeling shit.

I got DHT tested in 2023 as I thought maybe low SHBG means I’m aromatising so much that T > DHT just isn’t happening, and my DHT ended up being really low.

I took proviron at varying doses (25, 50mg a day) for a few weeks. All that happened is I lost lots of hair and felt dryer.

I’m going through periods (especially now) where I have quite significant brain fog (worse the higher my e2 is) where I genuinely feel retarded. I’ve always been sharp and intelligent, but at the moment that just isn’t there, and I actually struggle to speak and fuck up a lot of words and sentences because of it.

I really don’t know where to go - any advice from anyone who’s gone through the same thing or is more knowledgable than I am would be so, so appreciated.





Maybe stupid but have tried just quitting everything for a couple of months?

I started a couple years ago because I’ve always felt way behind RE masculinity. My T was 250ng/dL - if I come off with or without PCT now I’m guaranteed to go back to that - although I’m almost certain my mental clarity will come back.

I’ve intentionally missed doses for a week before whilst I was on propionate to experience the sharp drop, and weirdly each time around the 4th day I had a libido boom (only a few hours) and felt pretty good.

How would coming off be a resolution when my base levels are so bad?

What is your age and current body fat levels?

I’ve always had good aerobic fitness levels as I’ve been active for years.

I currently run once a week all day (Airsoft) and try to do 1 or 2 other cardio sessions a week. Apart from that I do work in office but my cardio fitness has never been much of a concern.

As far as I’m aware (All health markers apart from hormones have always been good) My neurological health is good. I’ve always been sharp mentally.

You’ve likely demonstrated testosterone isn’t your core issue with aforementioned symptoms. I’d suggest keeping your hormone levels stable and pursue other causes.
