Lower Energy Levels, Lack of Libido, Lab Results

Been reading through the forum and thought I would see what you guys thought about my case / lab results.

I am an avid runner and cyclist. I run 40 to 60 miles per week depending on what I am training for. I bike 60 to 120 miles a week depending on what I am training for. I also strength train 4 days a week.

In terms of my diet, I am very disciplined and eat a whole foods diet that is rich in fiber, fruits and vegetables, whole grains, and lean meats / cold water fish. I watch my hydration levels and electrolyte levels religiously as well.

Over the past 2 years I have noticed a decline in my average training pace on most days. My overall strength levels have not declined, but I don’t get the same results either from strength training I used to. I can still get erections, but my sex drive is very low. I never wake up with an erection. I have tried reducing my training but it had not effect. I don’t drink heavily (never more than 1 or 2 beers or a glass of red wine) My resting heart rate is still low (40s) and my maximum heart rate is still up around 190. As a performance comparison, 2 years ago my race times would be 1:27 to 1:29 for a half marathon and today its more like 1:30 to 1:35. My blood pressure can also get very low if I am sitting for a long time. I discussed all this with my GP and she ordered a lot of lab work. So here it is:

Might as well post ranges for everything…The boys will be asking for them.

Everything showed up as normal in the results with the exception of the following:

RBC - low
HCT - low
HGB - low

TSH - High
Free Testosterone - low

My doctor said I was mildly anemic and mildly hypothyroid.

My doctor prescribed 75 mcg Levothyroxin for my thyroid issue and told me to take an iron supplement to address the mild anemia. She did not want to treat the low testosterone. However, the testosterone numbers look low to me and I am wondering if that is the root cause of the mild anemia and being hypothyroid - in addition to the lower energy levels and low libido.

Thyroid affects testosterone, so correcting thyroid may be enough to normalize testosterone.

Therefore, if I were you I would first treat the thyroid. Give it a few months to stabilize, then retest testosterone. Only consider TRT if it is still low then.

WAIT:

We sometimes have hypothyroid cases here from guys who have not been using iodized salt who are iodine deficient. What is your iodized salt history? We don’t see doctors asking about basics like this.

I see where you are!

Very hard to read. Suggest you past into a text editor and see if the format is not messed up.

Total Protein is low and
RBC - low
HCT - low
HGB - low

Are you getting enough red meat? Either that or you have a GI bleed. Any food sensitivities or digestive conditions. You can rule out/in a GI bleed with an occult blood test, poop smear test. If negative, your “thin blood” might be a result of low T and TRT would then resolve that. You can also test serum iron and ferritin.

Would also be a good idea to screen for thyroid antibodies. Thyroid autoimmune diseases can be a result of selenium deficiencies.

I only eat red meat a few times a year so I would imagine that was the reason for the mild anemia. Almost all my protein comes in the form of white meat chicken, fish, legumes and greek yogurt.

I take a multivitamin and my diet is like a model diet in terms of the views of a doctor or dietitian. On a typical day I will eat whole oats with fruit for breakfast, an avocado as a mid-morning snack, a spinach salad, fruit, almonds, plain greek yogurt with berries, and smoked salmon wrap for lunch, fruit as a mid afternoon snack, chicken or fish with roasted vegetables and brown rice or quinoa and a green salad for dinner, a melon or fruit as a desert, and a whey protein shake before I go to bed. I never eat fast food.

As I stated earlier my doctor prescribed 75 mcg levothyroxine and wants me to take an iron supplement. She wants me back in 4 weeks to check again. So far that is what I am doing. Do you think that is a good course of action for me?

Get an occult blood test. Low iron does not explain lower protein level and depth of HTC. Meanwhile you do need iron and T will help boost RBC count. Have you been feeling a reduction in oxygen transport?

Still need you to check your body temperatures now and a these can be a good guide to your thyroid med dosing. Better than simply looking at how your labs respond. Also good to know your fT3, fT4 levels. There is a lot going on that TSH does not address.

You seem to be looking for reasons to dodge my suggestions.

I am assuming that your total cholesterol is low. Cholesterol is the chemical base for your steroid hormone cascades. TC<=160 is associated with increased all-cause mortality. Most docs have been programed by drug reps to think that lower and lower cholesterol is good - WRONG!

Suggest that you eat steak and beef liver. You need the iron, protein and cholesterol.

Thanks for the suggestions. My total cholesterol last time I had it checked was 195 and my hdl was 82. So my ratio was 2.4. I have my cholesterol checked once a year and the highest it has ever been is 205 while the lowest it has ever been is 183. My HDL is always very high so my ratio is always good.

I will email my dr about an occult blood test. I still train hard, I am just a little off from my peak. I will take my temps tomorrow in the morning and through the day. I know its always been the case in races and duathlons that I am most competitive in races when its really humid and hot out as I can handle that better than most guys. Although, I have also sweated out over 10 pounds of water weight on a long run in the summer. In terms of cold tolerance, I get out and run in temps all the way down to the single digits, but I always have trouble keeping my hands and feet warm.

I have also read that its not uncommon for endurance athletes to test slightly anemic because of their increased plasma volumes. idk

Last time I had some comprehensive lab work was back in 2008. They did not check T levels or thyroid then though. I noticed I was borderline anemic back then too though.

One last question. Hopefully treating my hypothyroidism will bring my testosterone and free testosterone levels up. However, assuming when the dr has me back for more lab work in a few weeks and my T levels are still low, should I talk to my Dr about maybe prescribing me something like Clomid rather than TRT?

If you do TRT, you might not be able to compete with drug testing etc.

Thyroid should be your main focus. Have you read the thyroid basics sticky?

You need selenium in your vitamins for proper thyroid function, otherwise autoimmune disease can occur.
