I’m hoping someone can shed some light on what to do next. For the last two years I’ve experienced the symptoms of low testosterone: Low libido, erection quality, energy, mood. However every time I got my total testosterone tested it was in good shape. So I simply put it down to getting older. However over the last six months it has gotten progressively worse. My oestradiol has consistently been below Reference range. So I hypothesize my SHPG may be too high. I went down to the states, I’m from Canada, to pay for a free testosterone SHBG test. I am 40 years old, 180 pounds, very good diet, exercise a lot. And otherwise healthy shape. My liver values are good, and I’m not on a ketogenic diet. My GP essentially said your total is good so therefore you’re good. I have a TRT clinic ready to give me TRT, however I want to make sure I’m doing the right thing moving forward.
This low estrogen can lead to osteoporosis and not many doctors are aware. You GP is clearly fixating on your Total T and completely ignoring the Free T, estrogen and bioavailable testosterone.
Your GP just doesn’t understand male hormones. So going back to him is just a waste of your time.
Being in Canada is going to be a challenge to find a doctor educated and trained in hormones, because there just isn’t many doctors up to date on current medical literature.

It’s doubtful you will recover naturally from these low Free T values, sure men have a little bit of influence in increasing testosterone, but not to the degree of going from rock bottom to high normal, especially considering T is already on the decline at 40.
