On the scrotal application part? I’m pretty sure that is what your asking. It is from my doc as that is what he uses himself and the cream is designed for by the pharmacy. I don’t know about the intratesticular estrogen part. I’m not sure if I agree with that. As intra, AI supposedly isn’t supposed to be stopped and it has completely crashed mine.

That is where my pharmacy intends it to be applied but im not sure if it has to be specially formulated for that. I believe creams were designed for that area though being that alcohol based would dry quicker and be slightly more convenient.
Labs were taken 9hrs post application, when cream peaks in about 2 so probably peaked even higher. So still really high.
If you mean based on protocol adjustments I have been same on cream for 5-6 months so I think I’m done adjusting in that sense.
Pretty sure I’m elevated and going to stay high and not drop into range anytime soon.

With cream the DHT levels peak 2-3 hours after T levels peaked. So maybe 5 hours is good.

No shit? What I have is 30ml of 200mg/ml in a transdermal base, that’s literally all it says.
So, I started the cream based on Defy’s recommendations and the ones here. The difference is that Defy absolutely does not want me applying to the scrotum, only to the deltoids and inner bicep. Empower pharmacy begs to differ and states that I should apply to the inner thigh (more vascularity apparently?).
Is this protocol doomed to fail because Defy has me applying it to areas which will lead to higher estrogen conversion and poorer absorption overall? I could go rogue and put in on the scrotum anyway but if I do end up with intratesticular estrogen I’ll need Defy’s assistance and they may not be willing. Thoughts?

Defy also gave me a hard time about scrotal application. They told me I could do a max of one click to the balls daily. I’m not on cream anymore, but while I was, I was just going rogue and doing my own thing. I didn’t see the logic in applying to other areas instead, and still don’t.
Switch clinics or who cares what they say. Just do it. Did you read the supporting material!

What are you scared of bro? I have mentioned that this is not a 6 week test. It takes very little effort to test this out. Otherwise sure apply to the body and get less T absorbed, which in turn gives you less free t and maybe less e2. But without higher free t you still won’t feel any better.
Goals less estrogen. Scrotul creates less estrogen. This is the best best option for this specific situation.

Chill bro, I read all the material I could find including what you posted here. It’s not a matter of fear but rather the wild card of intratesticular estrogen being created and personally having no idea how to take care of that particular problem if it arose. That being said, I cannot find any info on intratesticular estrogen apart from what I’ve been told by Defy.
Interesting they told you the same thing. Fun fact, I actually contacted Empower and spoke to a pharmacist who said that multiple Defy patients are doing the scrotal application. Not sure what the issue was with us.
Transdermal applications to the body reportedly are 10-20% absorbed, with more of a tendency to increase E2.
Transdermal applications to the scrotum are ~70% absorbed, with less E2 increase.
Either way, you’ll know if it is working by how you are feeling and past that, the follow-up labs will tell the story.
They are the only ones i have ever heard of saying about intratesticular aromatization with cream. Never read it anywhere either. I have had aromatization but an AI hits mine and drops it.
From i understand if it is aromatizing in the balls an AI shouldnt touch it.

Now HCG does because it stimulates the testes and I can understand that. The cream does not stimulate thebtestes to produce Testostorone.
All of the studies and pharmacology descriptions (whatever we call them) speak directly opposite of what your doctor has said.
It’s a shame that a medical doctor is making such statements.
