At 5’8" and 160, I think your problem is definitely both fat and lack of ab musculature. While you may not have a lot of fat, you also don’t have a lot of muscle, so what small amount of fat you do have is obscuring the small musculature of your abs. Your ab muscles have to show through the tendonous bands of fascia covering them that give them that segmented look. Imagine your ab muscles like a balloon, wrapped in a webbing. If you inflate the balloon (i.e. build the muscles) it will expand and poke out through the webbing, giving you the six pack look. Anatomically speaking, the rectus abdominus is actually covered with one vertical and three horizontal tendonous sheaths, meaning that everyone technically has an 8-pack. However, on most people, one of the sheaths is not as deep into the muscle and not as easily visible unless body fat is extremely low.

So, you probably are fairly lean, but you just need to build some more muscle to get those abs to show through.
