Help Needed with my Bloodwork Results

Hey! I’m a 22 year old male who’s been having trouble with bad libido. If my earlier libido was 10, my current one is like 3-4. I started noticing this 2 years ago but in retrospect it probably already started like 4 years ago.

Naturally, this is hugely psychologically stressful to me since I pretty much don’t have a sex life. As for the dick function, it’s a bit impacted (but I can still get hard erections, I just don’t really enjoy it) but a few days each year I do get normal libido and then my dick goes bananas in my pants, so I know that I don’t have ED. Just wanted to clear that up so we focus on the right thing.

When I was 12 I got gyno and it didn’t go away. Had it operated a while ago. Dunno if this is useful info.

A good thing is that this doesn’t seem to be impacting my training. I’ve been training serious (ie heavy compounds movement, the right diet, etc) for a few months now and I’m very happy with the results.

Don’t have any trouble putting on muscle and strength. I did unserious and sporadic training for years previously, and even these shitty workouts managed to make me quite well-built. Now I’m getting compliments for being huge all the time (obviously not compared to you guys, lol).

I also have some issues with depression, anxiety, fatigue, bad sleep. As for the first 2, I had plenty of those even when I was horny so it might not be related to testosterone levels. They do react positively to lifestyle changes and attempts at personal improvement.

As for the fatigue and bad sleep I’ve had those all my life. It’s something I am wanting to look into. Maybe I have sleep apnea, who knows?

Here’s my personal data:

Age: 22
Height: 194 cm (about six foot four)
Weight: 114 kilos (about 251 pounds)
BF%: dunno but my guess would be 20% (I was fatter and hugely horny up until about 2005 so I don’t think this is the problem, even though I obviously want to cut)

Next is my bloodwork data. The thing that pops out to me is that testosterone is way low, below the doctor’s normal range even (and we all know how crappy and conservative the normal ranges are). If there is anything else that pops out, do tell me.

Also, I would like some input on what could be wrong with me. Testosterone shouldn’t just drop like this for a young male, should it? And why isn’t my training impacted? Why do I get a few times each year where my libido works for 1-2 days at a time?

Do I have some kind of disorder/disease? What can I do? HRT is my last option, as I don’t really enjoy the thought of sticking needles in my ass for the rest of my life. I have a meeting with an endocrinologist booked so I need a list of stuff to bring up with him.

Though your free T4 is within range, your TSH is out on the high side. This indicates possible hypothyroidism. Your body may not be converting enough T4 into T3. Get T3 tested. KSman is one of the most knowledgable guys to get a hold of to review your bloodwork.

You have low Test. At your age, it may be your pituitary.

Yeah, this would be better over in the ‘Geezer’ section.

Try to get the LS and FSH lab ranges. The numbers do not mean anything to me without.

Your mid range estradiol may be dominating your FT.

I assume that your testosterone is total, not free.

Your TSH is definitely of concern.

Do you have any skin, nail or hair problems? Do you feel cold or chills more than one would expect?

Any vision changes?

Treatment options and expectations: What country are you in?

Many symptoms of hypothyroidism are the same as hypogonadism. The sex hormones and tyroid hormones both need to be managed.

You may have a pituitary problem. The lab ranges fro LH and FSH will be helpful in that regard. With young men, one needs to consider damage to the pituitary glad from a blow to the head or from a growth.

Thyroid: Do you consume iodize salt? Sea salt does not have iodine unless iodized. Fish also provides iodine. Does your thyroid feel large? If you have your thyroid palpated, that might affect thyroid lab results. Don’t get lab work done right after. Yes your FT4 number is good, but the high TSH could indicate that the thyroid is needing extra TSH to get there in compensation for low iodine levels. But these problems can also occur with adequate iodine intake.

In the time frame of things going wrong, can you recall a blow to the head or whip lash?

Dry skin, especially after showering.

I have to blow my nose several times a day, especially after eating. Had this for ages now, like 2 years. Might be relevant?

Vision has been getting a bit blurry. Might just be myopia from excessive computer use.

Guess I’m going to need a new test. The doctor I went to wasn’t all that experienced with endo stuff. I basically gave him a list of stuff I wanted checked and he put in the request (and I’m just a layman in these matters). I shall make sure to emphasize the LH and FSH ranges next time…

No, salt in my diet is mainly from sea salt.

I get like 4 caps of fish oil a day lately.

How can I tell?

In the spring of 2006 I fell into a wall while drunk. Not a major blow I think, had some aches in the bone of the forehead for a few weeks. But my horniness was shitty at this time already, so I think we can rule it out…

Get copies of your lab work, always and keep these. That will have your range. If not, make them find that info.

The thyroid is in your throat, you can feel it but might not know what you are feeling. You doc should be able to check that. Does your neck appear thick around the ‘adams apple’ or not symetrical?

A growth that presses on the pituitary and causes low T, can also press on one or two optic nerves and create vision disturbances.

Stay on that TSH problem. Tyroid problems can be serious.
