First 4 Weeks Blood Work Results

So I’m 4 weeks into my TRT journey. And took a blood test, to see how things are going

Testosterone Result 25.10 - Range 8.64 - 29 nmol/L
Free Testosterone Result 0.761 - Range0.198 - 0.619 nmol/L
Sex Hormone Binding Glob - Result 14 - Range 18.3 - 54.1 nmol/L
17-Beta Oestradiol Result 166.0 Range 36 - 146.8 pmol/L

Dr asked if I was having symptoms due to the high estrogen.
Mood isn’t bad, up and down
Libido is shot to shit
Gained 2.8kgs
Sleep is poor

My prescription is - although today I’ve moved to Cypionate, as I was getting a reaction on the Enanthate

So for Enanthate (250mg/ml) - you are prescribed 0.20mls twice weekly. 0.20mls is equivalent to 50mg of Enanthate so you would be on 100mg of Enanthate per week.

The HCG is 5000iu - on your dose of 0.20mls twice weekly after mixing with 2mls of BAC water is equivalent to 500iu per dose - so 1000iu per week.

Waiting for further feedback and if they’re going to make a change or just get me to stick with and assess again in another 4-6 weeks. As I realise its still early and rapid changes this early into things probably doesn’t make sense

Your very low SHBG suggests some other health issues at play. I would imagine fixing that would help your e2 issue.

Steady states are achieved in 6 weeks, not 4 weeks. Starting a new ester effectively amounts to a dosage change, however slight, sending your hormones into an unsteady state for 6 more weeks, ups and downs are par for the course on injections for some men after starting TRT or changing dosages.

You should be checking labels 6-8 weeks.

This is likely why you feel like shit, drop it and focus on TRT in isolation!

HCG causes water retention, lower libido, ED, fatigue and mood problems for some men.

Your SHBG was most likely higher pre-TRT (25), since androgens typically suppress SHBG.

It was actually 13 pre TRT

Im seeing a urologist and having a bunch of tests and scans as part of all this process

And the bloods after 4 weeks was at the request of the doctors administering TRT as i was complaining of some sides

People with type 2 diabetes/metabolic syndrome usually have low SHBG. Lower SHBG is strongly associated with many diseases including cardiovascular disease, cancer etc.

I’m a type 2 diabetic, my SHBG pre-TRT was 11, and now it’s 24.

Is your suggestion i drop the HCG altogether or lower the dose? SHBG aside. It sounds like HCG could be my problem and if so. Ill speak to the doc about dropping it

Yes, I think starting everyone out on hCG amounts to a one-size fits all approach.

TRT needs to be tailored to the individual’s needs.

It’s best to start out on TRT in isolation, add stuff later if still having issues.

By adding too many moving parts, it’s tough to determine what’s causing the issues.

Im going to speak to the dr tomorrow

Fertility isnt that important to me at 42. But should something drastic change. Could HCG be added back in to help or just come off TRT

Prob the case of thers always the risk and its individual

Strong thumbs up from me. Take the smart approach with one drug at a time. Give enough time for steady state levels and then analyze, add/deduct etc.
