Diabetes, Hypertension & ED (KSMan)

I could use some help or suggestions:
I am 5’10" about 260lbs with diabetes and hypertension and 59 years old. I have major ED and nothing seems to be working. My T was very low and my original doctor gave me a TRT shot once a month. I was not doing any better, so I called a compounding pharmacy and the pharmacist recommended a doctor who he said knew his stuff with male hormones.

So, I went. He tested me and originally put me on compounded cream. None of the gels worked in getting my levels up, so we resorted to one .75 IM injection a week. That brought my T levels up My Estradiol before the injections was 42.5. After my T levels went up, my Estradiol levels were 116.7, so he added Anastrozole 0.1mg capsules taken 1 a day for five days, skipping two. I have noticed no difference with libido or ED, but did have water retention on my legs, so I stopped the TRT and that problem subsided. So, I am wondering where I go from here. Here are my lab numbers:

Estradiol pg/ml 7-6 - 42.6: 116.7
Testosterone 240-827 ng/dl: 907
Cholesterol: 215
Triglyclerides: 414
HDL: 31
APO-A1: 131
APO-B: 129
APO-B/APO-A1: 1.0
VLDL: 83
Direct LDL: 132
Chol/HDL: 6.9
LDL/HDL: 4.3
Total Non HDL Cholesterol: 184
A1C: 9.8

Original Bloodwork before TRT IM Injections:

DHEA-S: 76.8
Vitamin D 25-Hydroxy: 35.7
Triiodothyronine, Free, Serum: 3.3 pg/ml
PSA: .94
TSH: 1.60
Total T3: 1.16
Total T4: 7.01
T-Uptake: 1.08
FT4I: 6.5
Estradiol: 22.7
FSH: 3.1
LH: 4.0
Progesterone: 0.18
Testosterone: 144ng/dl
Insulin: 7.2

I would really appreciate any suggestions any of you might have in order that I can get my libido back and ED gone, too. And above all else, that I might feel better. Thank you for your time!

I am going to try SubQ TRT with EOD .50iu’s and see how I do. I am wondering how much of the Anastrozole I need to take and should I be taking HCG?

You need to drop your E2 levels - I use 1mg per week anastrozole and it brought me to 23 E2

Thank you! Do you think that is what was building the fluid retention? From what I read on this site, that seems to be the case.

You need to get better controls of your blood sugar. Your a1c is horrible. Most likely your insulin levels are high and that is causing as mush water retention as the e2 is. You need to controls something here. I would continue the anastrazole at some level while reducing the testosterone. Begin a strict low Carb high fat diet to lower insulin. Monitor BP and blood sugar to normal levels before attempting to increase testosterone. Just getting e2 into normal range increases t levels.

Take care, you have much on your plate!!!
