Looking to see what thoughts / experiences some of you may have had in regards to Clomid to increase sperm counts / fertility
Long story short, naturally low testosterone levels and thus terrible sperm counts, hovering between 1-5 million/ml.
Have been on Clomid 50mg every day for 6 weeks now and just got first lot of testing done.
Before clomid
LH - 4 (range 1-10)
FSH - 6 (range 1-10)
Testosterone - 5 (range 10-33)
Free T - 155
6 weeks on clomid
LH - 17
FSH - 20
Testosterone - 20
Free testosterone - 577
These results indicate my balls are responding to the Clomid well and making testosterone which is great, but unfortunately 0 improvements on sperm count.
Both sperm counts before starting and 6 weeks in are a measly 1 million / ml.
The big question here, do we think 6 weeks is just too soon to see any results on the lab work and I should stick with it for another 8-10 weeks and retest ?

This is what I found on the internet:
Clomid for male infertility causes an increase in FSH and LH secretion. This leads to an increase in both sperm production and testosterone secretion.
Guys may need to take Clomid for at least 3-4 months to see the optimal effect. It takes 90-108 days from the time a sperm is made until it is ejaculated.

As @rusty_hammer above has stated 3-4 months before semen analysis after starting clomid. Anyone stating they got their wife pregnant after 2 weeks on clomid was already fertile
Also personally feel from experience that 25mg/day is more than enough to increase your sperm count, however if your having no side effects and e2 is okay then continue
A very important thing to know which I only learnt through experience, is that you need to take days off clomid every month in order to keep up its efficacy. Typically 25 days on 5 days off (my numbers dropped back to baseline from continuous use)
Look at taking a male wellman supplement or something that contains the same ingredients

Will continue to take clomid until it’s been a full 4 months, do sperm test to see where things are at and decide future options based on that.
So far so good on the 50mg every day, e2 level on most recent test was still okay, will retest this though when I get to the 4 month mark. Slight mood swings and seem a bit more inpatient but overall trying to stay in a good mindset on it
Wow that’s interesting about having to cycle off clomid. Definitely not something my endo had advised

Well it’s been 110 days and not great news unfortunately on the sperm count. Will post a photo below of what results 50mg of Clomid ED has caused, it caused a significant increase in my testosterone , LH and FSH levels.
Unfortunately this hasn’t carried over to sperm count though. Do we think any point in continuing Clomid for a bit longer to see if things increase?
My endo has suggested I now drop the Clomid completely, jump on HCG 500iu 3x a week to start with (and most likely increase dose age as required) and see how things go, with the option of adding in FSH after a few months.
Do we think HCG and FSH could still prove effective even though Clomid hasn’t worked in my scenario ?
Lastly, do you think the E2 level on the final bloods is too high and could be causing issues? It’s well over reference range but I haven’t used an AI or anything as yet.
It’s just my opinion, I think you’re infertile and it’s permanent.
Yeah it’s definitely concerning, we are going to look at IVF for our second child now. I’ll run the hcg to compliment that before changing over to TRT.
So you think I am primary hypogonadism? Even though baseline LH FSH were low-normal in ranges and not elevated ?
No, maybe mixed hypogonadism, which is a defect in the hypothalamus and/or the pituitary as well as the testes.

@Lamby22, drop the clomid dose in 1/2 and continue for many more months up to a year and you will assuredly have a higher count.
Don’t do the HCG + FSH thing = your LH and FSH is already sky high, far higher than injections could get you.
As pointed out, sperm takes months for mature from start. You are just starting out. Test again after 3-4 months.
Go check all the IFBB champs and pros they all have lots of kids.
