Does this kind of thing usually only happen in gear or does it happen raw too? I have a hunch it’s more common in gear because it increases blood pressure more. [/quote]

I am not sure how much a healthy person has problems with this. But if you have aneurysm in your brain you are not allowed to lift much anymore, because the pressure will pop the blood vessel. Risk factors for an aneurysm include diabetes, obesity, hypertension, tobacco use, alcoholism.
