Borderline TT, Normal FT, is TRT Suitable?

Hi there,

44y/o here. No obese, training 3 times a week. Moderate healthy lifestyle.

I have low mood, libido and drive. Also anxiety and cognition problems.
Labs show low, but within the range TT of 11.1 but good FT (i quess) at 22.6.
SHGB is 19. So the low SHGB corrects my low TT for my FT and i shouldn’t have these low T symptons?

Don’t remember E2, but it was on the lower side of the range
Is TRT suitable for me with FT of 22.6. or is it not worth with this level?

Yes, correct. But stopped. Labs are now about the same prior TRT

If you’re still going through low T symptoms, then it sounds like you’d make a good candidate.

I don’t know your lab’s ranges, but if your TT is in range (albeit low) and FT is normal, its probably not “medically necessary”, so it depends on your symptoms and whether your provider will prescribe TRT due to symptoms.

He already prescribed Enanthate. Bu i’m still hestitant to take it because i don’t it is is worth with mt FT level

Are you concerned of having too high a fT level? as long as your blood pressure and HCT aren’t having negative reactions, there’s not much else that Enth will affect negatively; elevated fT is rarely - if ever - a problem

Unfortunately, no consensus has been reached regarding the lower TT threshold defining testosterone deficiency, and there are no generally accepted lower limits of normal TT. This lack of consensus follows from the fact that no studies have shown a clear threshold for TT or free T that distinguishes men who will respond to treatment from those who will not.

The prevalence of hypogonadal symptoms increases with TT levels below 12.1 nmol/L (350 ng/dL). However, Zitzmann et al. have shown that testosterone deficiency symptoms may also be seen with TT levels as high as 15 nmol/L.

Meanwhile the number of CAG (cytosine–adenine–guanine triplet) repeats in androgen receptor differs in men and influences the androgen receptor activity. Hence testosterone sensitivity may vary in different individuals.

The same applies to androgen receptor gene CAG repeat lengths >24 in the presence of symptoms and normal testosterone levels may be considered as a state of preclinical hypogonadism.

If by lower end of the range means below 73 pmol/L or 20 pg/mL, this would be an estrogen deficiency. A normal healthy estrogen range is 73- 128 pmol/L.

All the normal ranges for hormones do not define “healthy normal ranges”. You can still be in range and still be at risk for disease.

Why did you stop, and why did you ask if you’re a good candidate? Clearly you were then and still are now.

The ref range over here is 120 - 180 nmol/l

These ranges are similar to the Roche ECLIA methodology here in the US.

Obese men can see estrogen higher than these ranges.
