You do not have secondary or primary hypogonadism, your pituitary gland is responding appropriately and is trying to increase TT so FT levels are healthy, but elevated SHBG is binding the majority of your testosterone and there’s not much you can do naturally to correct it except TRT.
Your estrogen is low as well, in your case it is low because FT is low, TT–>FT–>E2. The thing working in your favor when starting TRT is estrogen is very low, therefore balancing estrogen on TRT should be easy if you choose the appropriate injection frequency.
Technically your levels are in range and that’s why your doctors says everything is fine, but these are not the healthy ranges. Now the challenge is finding a doctor that is knowledgeable and understands the implications of elevated SHBG.
FT is low, that is why you feel like trash brother. Your gonna be hard pressed to find a doctor that will put you on TRT though with a TT of 600 ng/dl.
Outside of the obviously crazy lab result and ruling out some obscure pathology, I would focus on addressing the above and take testosterone. Double your free testosterone and you will feel better.
It all goes to what doctors are taught in Medical school, in range is normal which removes critical thought. There are many men who would not feel good at your levels, but most doctors don’t know anything about normal hormone levels, in range is normal when you consider age. It’s not alright for a young man to have similar testosterone to an old man.

Your free T is fine. And there is a good chance that you improve freeT even more with some life style modifications such as weight loss. TotalT, freeT and SHBG are all associated with BMI.

Saying free T is fine means it’s not good.
Being 5’2 is fine if there’s nothing you can take to be taller. You can survive and live a decent life. If there was an injection that made 5’2 people average+ height every one of them would do it in a heart beat. This dude isn’t in the best of shape so he should sort that out before TRT but it would likely be life changing for him if that’s his true levels. Especially with the “symptoms” he listed.
Yeah i’m 6’1 and 185 pounds. I workout most days and mix up cardio and weights. My diet could be better for sure and i’m working on that diet but I wouldnt say i’m out of shape
Assuming that you are also irishlad1989, you do not have a BMI of 25, it’s 24.4 and this shows the pointlessness of BMI. Post stats, BMI is a useless item. That weight at that height would be pretty tricky to be fat at. I looked like a torture victim at that weight at 6’-0.
Thanks man. Honestly i fill like im in pretty good nick which makes it annoying that my Free T is on the lower end. Still not sure whether to go down TRT route though.
