Advice on Lab Results

Hello all!

I’ve been lurking for a while, figured it’s time to say hello. I have learned A LOT, and I’ve seen a lot of great advice on here so now I want some of it.

-30
-5’7
-36’
-195lbs
-Body and facial hair are decent
-I carry probably 80% my fat in my mid-section
-I feel tired all day, every day. No matter how much sleep. I’m also very cranky and have a short fuse. This has been going on for about 6 years
-No drugs other than anti-d’s in the past, and prevacid
-Lab results are below
-Diet is around 2200 calories a day, maybe less on non-workout days.
-I try to do more cardio, as weights are very difficult. Much of my strength has left me and I feel like I’m 80 yeras old
-My testes never ache, nor do I have fevers
-I might have morning wood once a month, that’s it.

I’ve been been to probably around 15 different doctors. Whenever my test has been ran, it’s always come back low and showing issues on the lab work but I was always dismissed since I have hair growth on my face and chest and don’t have ED.

You name the test, blood work, supplement, doctor… I have been to them. No one wanted to touch my lab work, or my symptoms… just said that it was part of getting old… I WAS 24 when this start… (F’n doctors!)

I have some lab work below. I have been on hCG from my current doctor. We’re trying hCG before anything else. I already went through one vial and got my results below. I’m not sure if this stuff is working for me. I’m doing 500iu (1/2 mL) every other day.

Test Total - 350 (348-1179)
Test Free - 6.6 (8.7-25.1) LOW
Free T4 - 1.3 (0.82-1.77)
Free T3 - 3.7 (2.0-4.4)
Reverse T3 - 23.8 (13.5-34.2)
TSH - 1.170 (0.450-4.500)
Estradiol - 8.2 (7.6-42.6)
Vitamin D - 70.0 (30-100)
Vitamin B12 -880 (211-946)

I just had labwork drawn again today. The labwork above was from 4 weeks ago. My doctor wanted to give it a little longer and see if anything kicked in. The above was after 4 weeks on 500iu every 3 days. After these results, my doctor bumped me to every other day.

I do not feel any better. My doctor has said he might want to try pellets, however I would like to try either Clomid or some type of injection before inserting something into my body for months at a time.

My main complaint is just that I’m tried. I feel like I’m dragging through mud all day long. Nothing has helped me in the almost 7 years of trying to figure this out. The only thing that keeps coming back low is my testosterone.

That TT and Free T are kicking your ass. You are 30 for cry-eye!

Hell, if nothing else, your E2 level could stand to be higher.

If I were your doc (and I’m not, for the legal folks out there), I’d stop worrying about the hCG and get you on some t-cyp… plus get a lab done to find out your SHBG, and LH/FSH levels.

Thyroid looks normal, you need to make sure your Pit Gland is sending the right signals and that the boyz are doing their jobs… as well as to make sure the SHBG conversion isn’t out of control (probably not, since both TT and FT are insynch and low.

I am in a very similar situation, Total Test 330 (300-1100) with LH being 4.3 and FSH 4.2. No idea what is going on because the thyroid looks ok. Waiting on DHEA-S, Estradiol, and prolactin. Has your doctor brought up the possibility of a vitamin deficiency or iron overload?

You do not appear to be responding to the hcg. If you are sure it is quality product (it is from an American pharmacy right?) then the diagnosis looks like primary hypogonodal. There is only one answer to that: exogenous T.

Note that this means Clomid will be of no use to you.

You can choose between gels, pellets, or shots. Shots are the overwhelming favorite on this site.

Yes, I’m thinking the same thing (about the hCG). I do not believe it’s doing a damn thing for me. Hell, I have had already one entire vial, and started on the next and feel nothing. I don’t have the labs with me, however I do remember my SHBG being on the lower end, and my LH/FSH being very low (not marked as low, but close to it). This was done when my Total T was 319 (390-1200) and Free T was 74 (47-225). Now, I am going from memory on these, so they could be off by a few here or there, but that’s what I remember.

Yes, he has, as well as past doctors have. I have been check for every deficiency possible. My ferritin levels and iron saturated % were perfect.

Just a little more insight onto some tests.

-Full Panel Allergy testing
-2 Sleep Studies
-24 Hour heart monitor
-Pulminary chamber testing
-Home tested for enviornmental toxins
-Complete blood work from infectious disease doctor (I actually passed out from all the blood they drew
-4 saliva tests. I suspect adrenal issues at first and went on Wilsons forumuals. I spent $1,000 on his stuff. Followed a strict diet, workout routine (just walking) slept 9 hours a night, and took all the supps he recommended. Never felt any better, though my saliva tests showed I was doing great.
-4 different endo’s for various testing
-9 different anti-depressants, plus 3 different stimulants (all did nothing for me)

I take a daily multi from ON, 5,000iu’s of Vitamin D3, 2,000mg’s of vitamin C, and some adrenal caps (still, just to make sure).

Sorry if that seems like a bit much… lol

Thanks for the heads up on the Clomid. I was going to ask for what I’ve seen (from you a lot) people talking about a SERM restart. So, that is worthless at this point?

About it being from an American Pharm, I cannot confirm at this moment. I believe it said USA on the label, however it’s given to me by my doctors office. They mix it there in front of me and send me home with it.

I have tried the Gel (Androgel 1%) in the past for a few weeks and felt nothing from it. My T results showed nothing from this. My doctor said (I really think you’re fine, just get more sleep)

What I meant from an American pharmacy, was if it was legitimate medical grade HCG. Unless your doctor is operating out of a mobile home in the middle of a desert, it is likely this is good to go and you just are not responding.

If that is the case, CLOMID and SERM restarts will be futile.

HAHA! Nah, he’s a legit doctor, working out of an office. They have to check my ID each time I pick the stuff up, too.

Well, lab results are back. My doctor said that the HCG didn’t work. My total and free test levels dropped even lower. They now said I’m primary and need to start TRT. I do not have the lab work with me now, she is sending me a scanned copy and I will place that up later.

They would like to do pellets, I asked for injections. Reason being, I want to make sure this helps me feel better (energy, alive, less groggy and cranky). If this does, I would be happy to do something that is only 4 times a year, but I know it’s pricey.

My question is this…

I’m only 30, and have no kids. Going on something the rest of my life… I’m cool with it, but what about the chance of having children? Is my sperm count lowered now due to the low test? Will increasing test help my production? I forgot to ask the doctor… lol

HCG will help maintain your testicle function and may or may not help with spermagoesis (I butchered that spelling). Being primary, it is hard to tell and a bit trickier. You may be at sperm count = 0 before even starting TRT depending on the severity of your testicular dysfunction.

HMG is also an option for increasing your splooge count. This stuff is VERY expensive though.

Have you even had a sperm count/fertility test done to see what’s up? If that is important to you, I would recommend getting it done before starting TRT. Otherwise yo uwill complicate things.

No, I haven’t, and I’m doing just that. The FDA approved home sperm test kits. I’m gonna pick one up and see what it says. If it’s good, I’m going to look into freezing some of my boys.

I’ll post of the labs later, still haven’t gotten the fax with the lab work on it.

Cool. what home test kit are you going to use? I may pick one up myself and see what’s up.

I’m no expert but HCG causing your test to go down and with that super low E2…you are primary all the way and need TRT. Having never tried pellets injections seem more straight forward to me but maybe a pellet person will chime in and give his opinion having used them.

Your E2 is probably just really low because your test is so low.

At least I know why I feel like shit all day, now. Just hope this stuff works.

Agree with everything here 100%

Just got my results back from the fertility clinic. Sperm is good. Sperm is going to be frozen next week, before starting TRT.

Question, is this. If my total T is under 200, my Free T is almost at ZERO, and HCG did nothing to help either of these labs/level should I have a good sperm count?

Yeah I started with a tT of 150 and have been chasing around a 2 1/2 year old for the past 2 hours trying to get him to bed. His 7 and 5 year old brothers are already in bed. Conception for each child consisted of unprotected sex ONE time! I’m presuming my sperm count and morphology is normal in spite of that awfully low tT.

While low T levels do affect many men’s ability to produce healthy sperm it seems this is variable.
