46 year old with PSA Increase Over 18 Months on TRT

PSA has been steadily rising over the past 18 months. Baseline was .68 and is now at 2.1. I know there is conflicting science out there, but I’m pretty spooked about this since usually there is a spike in PSA, but then a leveling off after starting TRT. I have some urinary symptoms which urologist has prescribed Flomax and Cialis 5mg/day and both seem to be working. TRT doctor will not prescribe any more until they figure this out. Been off 3 now for weeks and feel like crap.

Anyone else been in this boat? Is this more common than I think when it comes to replacement side effects? All the research I have seen seems to suggest otherwise.

How old are you? What other meds are you on? Do you participate in sex that involves your ass (no judgement here)? Have you had a DRE? History or UTIs? Were you previously on propecia (finesteride)?

46 year old heterosexual (no ass play here). Had a DRE and showed mild inflamation. Had a UTI in my late teens. No finesteride and no other meds besides flmox and cialis after the fact.

So first off ass-play isn’t necessary homosexual but if you say no ass play then ok. Just know that if you do/did regulary shove anything up there, or your partner does, it can increase PSA.

Im assuming the DRE happened AFTER the blood pull for the PSA numbers? See above as any prodding of the prostate will increase PSA.

Have you been tested for a UTI or for a prostate infection? Taking any obscure supplements or black market shit?

No black market shit or supplements to speak of, other than protein powder. I try to use the cleaner ones with no sugar, but you know how that does. Urinalysis showed no UTI infection and yes, DRE was post blood draw. Doc put me on a couple weeks of Bactrim in case of infection which I finished 2 weeks ago.

And have you had another draw since finishing up the antibiotic?

Not yet. They want me to do it in a couple weeks, because it would be 3 months after the last draw. TRT office runs blood work every 3 mo.

Ask them to check your free PSA level while they are at it.

What was your TRT protocol? How high was T and E2.

Most side effects are dose dependent, and some are more susceptible to these side effects than others. High HCT, PSA elevation, etc

Your doc is doing the right thing. When it has cleared up and you are ready to restart consider to reduce your dose in agreement with your doc.

Have you been diagnosed with bph? I’m a bit older than you and my psa always runs between 1.9-2.5. Once it spiked to 3.4. My Dr diagnosed it as a prostate infection, gave me some antibiotics and it came back down. Another thing my Dr told me is to refrain from sex 72 hours before my psa. I do this before every test and it also helps.

No diagnosis as of yet. They wan to run PSA again. Been through the antibiotic course and haven’t been tested since.

Was on 200mg Cyp per week. Last blood work showed 826 total T, Free Test was 27.6, E2 was 42. I’m off everything 4 weeks as of today and feel terrible. No PCT.

Where are you located. Your doctor is correlating trt to prostate. Doesn’t happen. You need to contact a smart trt doc who can guide you. These other docs don’t care . They’ll take you off trt and make your PSA Rise even higher

St. Louis area. And I’ve already been taken off TRT by the actual TRT specialist/urologist. Said 2.1 is too high with a baseline of .68 merely 18 months prior plus urinary symptoms. Been to other specialists at Washington University St. Louis and they agree. They are concerned about velocity.

I know this is stressful but getting off trt is only going to make things worse.

A hormonalLy balanced man Is healthy and does not have prostate cancer: men who do get cancer are due to low hormones and etc

There are plenty of new studies out there showing that it’s better to leave a man on trt than to take them off.

I am from Houston, but I lived in STL for over 25 years. Your best bet is to find doctor Jeffery rutherbusch out of Florida. He has been doing this for decades and he does telemedicine. Stop screwing with non trt doctors they have never spend time actually learning about HRT. They are literally looking at a suggestion in the system and past knowledge. They aren’t going throghb the latest literature and trying to understand. Their simple answer is blame T.

Call dr rutherbusch he is well versed in prostate and men’s health. Since he’s in FL he can do telemedicine.

If for some reason you want to meet a doc in person I would go see tier1 out of chatanooagah TN or dr Jordan grant out of Dallas Texas (north of Dallas). I love all three of these docs and they all have become experts in HRT.

Otherwise stay off trt and stay miserable along with the other nonsense they want to do.

I’ve also read that psa can easily increase if you had sex the day before or within 48/72 hours of the test. Including other factors. I would abstain from sex, drink lots of fluids and eat healthy for 5 days and take the test again. This might be a fluke. PSA levels are like TSH levels. Extremely inaccurate. They fluctuate for the slightest of reasons and they have never been reliable.

Anyways that’s all I can say. I simply wouldn’t get off trt and I would find a trt doctor that actually gets it. I’m sick and they want to make me sicker? What gives.

Thanks. I can’t find any information for Ruterbusch’s office. All I can find on google is some interviews with him.

Your doctor may be a TRT specialists, but it in no way guarantees he’s a good one who is well informed. PSA will rise after orgasms and hugging puppies and kittens with 48 hours.

You could have a simple prostate infection and your doctors seems awful nervous for a “TRT specialists”.

Your PSA isn’t that high. Secondly, a higher PSA doesn’t mean cancer at all. Third any prostate stimulation including organism raises PSA for up to 48 hours. Fourth relax. Get back on the T and enjoy life. Fifth, lots of orgasms are good for the prostate so cum once a day if possible.
