I have secondary hypogonadism and finally found a doctor willing to prescribe cyp T.
The first couple weeks of TRT were amazing (100mg/week), but a month later my libido and energy have since fallen to dismal pre-treatment levels and the new injections barely have an effect.
The doc doesn’t know about the testosterone-estradiol feedback loop or the advisability of an AI (or hCG for that matter) but I managed to convince him to prescribe an E2 test. My pre-treatment E2 was 26 (Labcorp range: 7-43).
Now I have to decide when to go in for the blood test. If I can show him a dramtatically raised E2 count, he’ll probably heed my request and prescribe an AI like Arimidex.
When in my weekly schedule will my E2 levels at their highest? A few days after the shot? Is it relatively stable after 4 injections?
T peaks about 1.5-2 days after injecting. E2 levels are a balance between removal by the liver and fT–>E2 aromatization. So E2 probably peaks day 2-3.
Nothing is stable with weekly injections. The T peaks from weekly injections also increase E2 above what the same dose of T would be in EOD/E2D or E3D delivery.
Your complaint is very typical of estrogen poisoning.
If you cannot get Rx, you are then forced to self medicate.
If doc is skeptical, explain that he will get to see how you respond. For me, adding anastrozole was a change of life. For you at 4 weeks, you have not slipped far, it can be worse.

Thanks - Haven’t been able to implement the full TRT protocol for the typical reasons as of yet.
