Looking for some input on my current TSH. My last set of labs I was at a TSH of 4.2 ref range .45- 4.5
My full thyroid panel in May prior to TRT my TSH was 2.9
T4 Free 1.43 Range .82-1.77
Rev T3 Serum 18.5 Ref 9.2 - 24.1
Thyroglobuin Anti body less than 1.0 reference range 0.0-0.9
thyroid peroxidase 10 ref range 0-34
Triiodothyronine (t3) free 3.5 ref 2.0 -4.4
I do not have any symptoms of being hypo ? I actually run hot since starting TRT and sweat a ton. No unusual lethargy… no weight gain… no hair thinning… and I enjoy the cold.
Was my thyroid healthy in May prior to TRT and what could cause the jump in TSH from 2.9 to 4.2
I have my follow up with defy tonight should I ask any questions or is there anything natural I can do to lower the TSH?
I would not make any decisions based on 1 high tsh lab.
Repeat tsh, free t3 and free t4.
My tsh was also elevated starting trt a couple weeks ago. I am also “running hot”. I feel like I’m having hot flashes multiple times during the day. Could this be a symptom of improper thyroid function?
I have not had the hot flashes. I am just hot in general. I think it is an increase in metabolic rate. If I am doing cardio I look like I just jumped in the pool and got out.
We have a couple of other members report an initial high TSH after starting TRT, then some time later it dropped back down to baseline. My body behaves strangely during the time my levels are in flux after a dosages change, this is when I encounter mineral and vitamin deficiencies.
I have tested vitamin levels when levels are in fluc and when levels are stable and vitamin levels are always higher after 6 weeks when levels are stable. These large spikes in testosterone and estrogen seem to cause a spike in how my body uses up mineral and vitamin resources.
TSH is such a poor biomarker, one guy can have a TSH of 2.7 and be full blown hypo with all the classic symptoms and another guy has a TSH of 4.2 and is relatively fine with minimal or no symptoms at all. Maybe the fact here is how long the patient spends at this TSH range, if it’s only briefly he can come out of it feeling fine.
The difference between spending 70% of the time with a TSH of 4.2 and spending 30% of the time at this level is going to produce a different outcome for a person because it would mean your thyroid hormones are higher longer.
When doctors look at testosterone and thyroid levels, I wonder do they consider the consistency of levels day to day, week to week. I bet if you had a way to test using a device top see where your levels are 24 hours a day, you would get a clearer picture of what’s going on because two separate tests isn’t really enough to paint a clear picture.
I’ve seen guys do multiple testing and caught the thyroid redhanded with abnormally low levels on only one of many set of labs.
I had my consult last night and the doctor did point out that my TSH was at the upper limits. He was not too concerned and ordered a complete thyroid panel in 3 months to see what is going on. I do not have any symptoms of being hypo so I am hoping this number drops.
At a 4.2 TSH I have felt great on TRT… so I am definitely hoping this is an anomaly or maybe just my body and my thyroid isn’t giving up.
I got that when my estradiol was low. And free t as well. You taking an ai?
My TSH level has also flared up quiet significantly during the first couple of weeks - going from 2.52 (lowest I’ve ever had in the last couple of years or so) to 5.56 - mind you I’ve had subclinical hypothyroidism for years, so my TSH levels were always around 4-5.5 . Since this came with a number of symptoms, my endo and I decided to try a microdose of levothyroxine (25mcg). I’ve only been on it for 2 month-ish but it is definitely working - more energy, lost weight so all in all it’s great. I feel that it complements my TRT protocol quiet nicely.
Thank you Alpine, what were your symptoms ? I was always insanely lean in my 20’s and had crazy energy but when I hit my mid 30’s it seemed like I could no longer cut fat and my abs sort of vanished. I found that around 2-3 pm when I am done work I get sleepy but once I start my workout it goes away. I do find myself falling asleep at around 830-9 at night but usually attribute that to how hard I workout. Since starting TRT I pretty much recomped my body but still get those bouts of sleepiness around 2-3pm.
Hi mate. I had extreme fatigue - I was quiet literally falling asleep at 6pm no matter the rest before. I also had trouble with loosing visceral fat as well, even when with fully optimised T level, had bad mood, and I it was the main reason why I wasn’t able to metabolise fat properly - I think this is why my lipid profile looked as bad as it did on a (nearly) perfect diet. I’m having my blood drawn on Friday and I’m dying to see what has changed ever since - I’ll definitely share the results.
No I’m not but I just restarted trt a couple weeks ago so I’m hoping it’s just my body’s reaction to something new.
Usually need to wait a good 2 months. What’s your protocol?
200mg once per week. I imagine my system is just in a state of shock.
Have you ever tried breaking the dosage up ? I think 200mg at once would give me all sorts of issues.

I have but I’m trying to wait at least 6 weeks before I change anything. If the flushed feeling for the first couple days post shot doesn’t go away I’ll go to 2x/week dosing. My shbg was top of the range so I’m probably not far off from what I’ll need long term to get my free T up.
What was your SHBG I started at 64 and am at 62. Feeling amazing at twice a week dosage. 160mg

It was 53 (16.5-55.9). That’s good to know, I thought 200mg/week sounded a little high. I figured it was just the opposite approach to my first trt experience. They started me too low and were very slow to raise the dose. Now that I’m doing injections at home I’ll have the freedom to lower the dose or administer it how I like.
