TRT But Still Lacking Urge for Sexual Desire at Times

Hi Guys,

I’ve been on TRT for a while now and take the following meds regularly.

Current meds I’m on:
Test Cyp - 0.3ml x 2 times a week 200 mg/ml (so thats 60mg twice a week)
Anastrozole - 0.25mg x 2 times a week

I’ve noticed that I have been lacking in sexual desire most of the time. I can still get hard and erections are pretty solid but sometimes the desire to do the deed just isn’t there, until the act begins.

I’m confident that this is a TRT thing as I have noticed one strange factor, my desire picks up the further I get from my shot.
A few guys have suggested that it may be due to me using Arimidex too much and that cutting it and increasing my estradiol would help.

I tried this and it did help a little. However my estradiol shot up after about a week and half of missing the adex.
I also did some more blood tests and here are the results below.

Any ideas, on what could be diminishing my sexual desire/libido?

Is my Progesterone or Prolactin too high maybe?

Feb 2022 - 170 pmol/L Range 0-160 (pmol/L) - Without adex taken for a week and half- I did ask for sensitive test, not sure if they did it.
Aug 2020 - 73 nmol/L - with adex (0.5mg x 2 times a week)
Feb 2021 - 74 nmol - with adex (0.5mg x 2 times a week)

My last shot before this blood test was Feb 17 - blood tests were on Feb 20.
Height: 6’3"
Weight: 113 Kg
Age: 36
Reason for TRT: Removal of undescended testis in 20’s. Labs found my Testosterone levels to be barely in the range. Started TRT to get back to a good base.

TRT does nothing for the urge. That’s in your mind. Someone posted a link to this video, I’ve watched another that covers the ED issue. It’s worth it to watch and listen to this whole video.

I don’t think its just mental for me. Lobido definitely increases as I get further away from my shot.

That is a fairly high dose of anastrozole given that testosterone dose.

Is this also further from your anastrozole dose?

Were there negative effects from this?

You can get that test, but it is an unnecessary expense as the ECLIA test is sufficient.

It can be long CAG repeat lengths, or the level of activity at the androgen receptors. You could also be burning through a vitamin or mineral as testosterone increases metabolic rates.

You’re probably burning through something and as you get further away from your injection, then the mineral or vitamin level recovers.

For instance, someone on TRT uses up more iron than someone not on replacement therapy.

Unless you have an elevated C-reactive protein, the standard test is fine and may be more reliable since the sensitive test is dependent on lab technician skill.

What you are saying isn’t lost on me. When my E was close to100, I lost all desire to have sex. The medical says the urge to have sex is mental. However, your hormones alter your mental state.

Trt does Nothing for the urge? Good One.

More people posting about lacking libido.

Numbers look good. Decent protocol.

Maybe we need more fluctuation in test levels? TRT was originally given once a week or once every two weeks. Not daily shots.

More look should be given to gels and creams imo. Daily application (just not in an ester that prolongs release)

Only people having problems are posting about lack of libido, men not having issues in their libido department aren’t posting about libido difficulties.

If you go to a mental health clinic, you’ll most likely run into peope with mental health issues.

Yes but to be fair, this post aside, plenty of these posts are saying worse libido after starting TRT.

Yea I take the anastrozole the same time after my usual shot, so the longer I get from my shot, the longer I get from the anastrozole as well. Didn’t think of it in that way.

I’m going to cut the anastrozole down to even less than 0.25mg each time I do a shot, or maybe take it between shots. To vary it up, see how I react to that.

I didn’t experience any negative side affects that I could see from stopping the anastrozole and getting into the 170pmol/L range, maybe I was a little more moody.

Had a call with my doctor this morning so waited for that before replying.

He was concerned about the E2 jumping to 170 and wanted me to bring it down pronto, asked to see it below 80. I explained why I did it and was just testing to see if libido increased.
He didn’t want to lower my Progesterone or Prolactin and said that he prefers to leave them alone right now. Fair enough, I would have preferred to try something to help bring the drive back by altering them. But I’ll start with the changes to the anastrozole dose and see how that plays out over the next few weeks.

Doc also did mention the nasal spray and said that he had seen some good thing on having that keep the sex drive alive. Unfortunate that they don’t have it approved in Canada yet though.
He said that the up’s and down’s the nasal spray brings to the Testosterone may be enough to keep the drive alive. That getting on a stagnant pattern may be the cause of the libido leveling out after some time of TRT.

I will definitely be investing in taking some additional minerals and vitamin’s Systemlord.
That thought hadnt occurred to me. I did use to supplement B12 and B6 a while ago, so will get back into that and taking additional supplements.

Something is getting skewed on TRT.

I’d love to see results, libido wise, from daily applications that mimic natural test production (gels, creams, jatenzo) vs. injections which don’t mimic so well.

My guess, and it’s pretty uneducated, is that TRT → higher e2 → higher PRL, and both of those need to be in personal range in order to have good libido. I’ve seen guys post pages and pages of labwork, and I’d say maybe 30% actually have PRL tested. Most have no idea it even exist or what it does to men. Anyway, like the guy in this post, his PRL is over range. Not like tumor-high, but high enough to fuck with libido and orgasms.

My inexperienced guess is that if the OP is saying that :

  • “my desire picks up the further I get from my shot.”
  • “I take AI the same day I take the shot”

… Then, that will probably be a time where you have less testosterone and more estrogen ongoing.

Not the first case where I read that less T = more libido. I guess we´re not all equal.

Cutting the Anastrozole to half or even eliminating it could be a good starting point. This should be observed along at least 6-8 weeks as per most of the more experienced TRT members here. Give it a time, do labs after this point and “see” how you feel.

You are taking 120mg and your Estradiol numbers are 73 with 1mg of anastrozole a week?
You are one of those guys (like me) that convert T easily to Estradiol, and it shows with your LH and FSH numbers. Try 1.5mg of anastrozole per week for 4 weeks and then get your blood tested again.

You might be convincing me to try less frequent shots again. Nothing is helping lately.
