My younger brother was put on TRT and recently started breaking out. Mostly red inflamed bumps some white heads. I am not sure what his dose is at the moment but, was wondering if splitting the dose may have an effect or if it is dose dependant.

You should read up on sexual complications in people treated with Accutane. Just lower the dosage of Test and DHT will decrease.
It sounds like your little brother could use a hormone specialist who can manage side effects, doesn’t sound like his current doctor is up to the task.

Topical salicyclic acid. Oxy acne pads.
Splitting dose will help lower DHT spikes.
Don’t use Accutane.
