Hey guys - 47 y/o male here with a family history of cardiac issues. 6’3. Was 340 lbs in 2023 now down to 300 but with a lot more muscle mass (still prob at 32% BF right now).
been on test cyp (around 220mg a week) and nand Dec (just 0.2ML in the syringe each time I inject test cyp) since early December 2025 - so been on test for just over a month and a half now. Also on 4-6iu daily of HGH and weekly reta/cag along with mots c, 5amino and nad+ buffered. Seen good results with muscle definition and starting to see more fat loss but still have that stubborn tire around my waist.
I just got back my blood labs and I’m at over 1500 test - which freaked my doctor out bc he wants me on 0.6ml a week (160mg) which seems awfully low for adding real Mass. but it’s not like I’m “blasting” test either at 220 a week- my RBCs have increased to 55 and my hematocrit is like 54.5%. Given my family history of heart issues, this concerns me because I don’t want increased risk of blockages.
I have upped my water intake to help with the hematocrit level but is there anything else you guys would suggest? Feel ok other than water retention (knees feel a bit achy) but was considering adding 100-200mg of mast or primo a week to the test to also help with the estrogen from the high test even though I have doc prescribed clomiphene I’m taking now). But concerned they could make things worse
Any thoughts / suggestions? I have a way to go on achieving my goals (15-18% BF + more muscle mass) - thanks so much guys

You Dr is not worried about you adding mass. He’s concerned about your health.
Also deca shows as high test so that will effect your results. Does he know what your taking? If you have a history of heart issues/family issues you really need to tell him what your doing.
Water retention is from estrogen. Are you taking an AI? Your conversion rate is prob very high at your BF level.
I suggest you stop taking drugs and lose weight. You are no condition to gain mass.
Lower test to what your Dr wants. Real TRT dose, not what your doing.
Drop the deca
Take an AI to get estrogen in check
Donate blood to help hematocrit(temporarily)
Don’t even think about adding compounds. That’s not how you manage estrogen.

Guys - the doctor was the one who prescribed me both the test cyp and the nand Dec; I’m not sourcing it elsewhere. It’s all under doc orders.
what I’m saying is I am surprised that my test levels are showing above 1500 when the most test cyp I’ve taken is 220/week. Maybe it’s that high bc of the nand Dec and I can def curb that but I feel great. No malaise, no side effects other than what the bloods showed as a high hematocrit level (and even then it’s 54% and I’m not drinking enough water to stay hydrated.) I do bloods every month all under doc supervision. I was just asking why folks think my test levels are so high when I’m not blasting…
(And my doctor - who rx me the test and nand Dec and the AI- is the one suggesting I I stay between 0.6-0.8 ml of test cyp. He didn’t say come off of it even after my latest bloods.
This does not surprise me at all. 220mg/wk would put most people above 1500. Add the nandrolone and I’ll bet you actual numbers are over 2500.
You pretty much are blasting. This is not a normal TRT protocol. This is a cycle.
If this is how your Dr is treating you I would consider a different Dr.

Yep. Get a new doctor and do the hard part; diet, so you’re not clinically obese.
Now that I think of it, he’s not your doctor, he’s your dealer.

You are 47 years old, 300 pounds, family history of heart attacks, 32 percent body fat and your goal is add mass?!?
That’s not a great goal at any age with those stats, but in your late 40s it’s an awful idea. Being that heavy and having hematocrit that high is asking to become another family statistic.
How’s the blood pressure?
You need to give blood to bring your hematocrit down and start talking to an actual Dr.
