Possible Subclinical Hypothyroidism. Bloodwork 4-5 Months into TRT

I started TRT with HCG around 4-5 months ago, but my symptoms aren’t really improving even though my labs are pretty good (or at least a lot better than I started). Originally a very well sought after doctor said my issues were related to adrenal and thyroid, and I am starting to think this is the case to some degree for me. My body temp is always lower than everyone else’s in the room, I have intermittent dry skin, low (almost non-existent libido), ED issues and such like even with decent labs.

As mentioned in another post, I had high RT3 show up on one lab and I am adjusting my diet to try and lower that.

Overall, I do feel good though in terms of mental function on TRT and want to continue it as I am seeing improvements. I have been very productive (work wise) recently and feel better all over with the exception of libido. I have also noticed an increase in body hair and a bit more definition after the gym.

My remaining symptoms are as follows:

  • Low (inconsistent) libido
  • ED
  • Borderline low body temp
  • Low sexual sensitivity

I am currently on 20mg of test cyp EOD, and 100IU of HCG daily.

Here are my labs a few hours before my next injection was due:

Thyroid

Free T3 - 3.45 ng/L ( 2.1 - 4.4 )
TSH - 0.68 mU/L ( 0.3 - 4.2 )
Total T4 - 0.73ng/dL ( 0.61 - 1.12 )
Total T3 - 1.1 ug/L ( 0.7 - 2 )

Sex Hormones

17 Beta Estradiol - 41 pg/nl ( 10-50 )
Progesterone 0.42 ug/L ( < 0.2 )
Total Testosterone - 5.29 ng/mL ( 2.6 - 10 )
Free Testosterone - 11.7 pg/mL ( 8.3 - 40.1 )

Obviously my free testosterone is still pretty low, though I imagine on the day of injection it’s noticeably higher.

Curious to hear any feedback you guys might have on the labs so far, it does look like my free testosterone could be a bit higher on the trough though…

Your dose isn’t even close to high enough IMO. You’re going to want your free T to be 3-4x higher before you’ll really feel good. At least at the top of the range… I’d work on that first then look at Thyroid later if you still have issues.

How much are you on/what dosing schedule?

You are underdosed, simply increase your dosage.

Actually your peak and trough levels are pretty close because you are injecting EOD.
