Normal Total T but Low Free T and High SHGB

34 male, Got some blood work done. Total test was within range but free T was bottomed out. I’ve been having some sleep and sex drive issues the past couple years. I’m a healthy weight and my diet and exercise are pretty good.

Worth considering TRT with my results? It appears that most of my free T is tied up in SHGB but I don’t know what to make of this. I talked to a Dr. and he said no one really cares about free T. Also, got my cortisol levels checked and they are very high.

total test: 458 ng/dl (normal)
free test: 44.9 pg/ml (low)
bioavailable test: 100.2 ng/dl (low)
Albumin: 4.9g/dl (high end)
SHGB: 45 nmol/L (high end)

My total T was 770 and 740, but my SHGB was over 60. At same age, I decided to start TRT and have never felt better. What BF % are you and do you drink? Both can cause high SHBG. Neither were an issue for me and didn’t feel like I could naturally lower it.
