Normal Testosterone but Very Low Free Testosterone

I know you mentioned E2 being in range, but can you post the number and range?

Too much. Constant stress like that can actually drive your T levels down and the inflammation can really keep you at odd levels. I expected you to tell me exactly what you did, I wasn’t a lot different around the same age. Cut it back. High volume is fine sometimes, but not all of the time and it is very taxing on the CNS among other things and requires sufficient recovery. Less is more.

It doesn’t change anything because I knew you meant pg/mL, these levels are suboptimal whether the units are pg/mL or ng/dL.

Please do a full thyroid panel test

1 ng/dl = 10 pg/ml.

You cannot compare labs companies reference range to one another, you’ll give yourself a headache.

Well you cannot have 10 times more testosterone in one lab than the other right :smiley:

But I think you refer to the testing method. And if his is direct assay we cannot compare it at all.

As much as I know and have researched deeply this the best for free testosterone is to be calculate it and I will not measure it anymore directly.

If I remember correctly LabCorp has a typo on their units for Free T. I remember some posts about it a while back. Maybe that is adding to the confusion. Maybe not.

Ok here are a majority of my bloodwork results:

Testosterone, Serum: 508ng/dL (188 - 882)
LH: 6.3mIU/mL (1.7 - 8.6)
FSH: 3.6mIU/mL (1.5 - 12.4)
Prolactin: 4.7ng/mL (4.0 - 15.2)
Estradiol: 37.6pg/mL (7.6 - 42.6)
Testosterone, Free: 11.5pg/mL (Not Estab.)
Regarding FT, my primary care doctor told me that he would have been happy with a number from 50-250. He then referred me to an endocrinologist.

Lipid Panel and CBC came back with every single value within the normal range.

Which tells me there is no mistake, this really is pg/ml which is 1 ng/dl. This is insanely low! You must have an enormous SHBG. Can you test SHBG these days?

The experts in testosterone would disagree. Dr. Abraham Morgentaler states that men will benefit from TRT with FT levels 15>. You have the sick care reference ranges and the optimal references ranges.

Then you go to see 10 different doctors are you get 10 different opinions. It’s clear there is no established methods for determining whether or not someone is actually experiencing a testosterone deficiency or every doctor would arrive at the same conclusion.

He said 11pg/ml which equals 1 ng/dl. Thats why the primary care told him he wants to see it in 50-250pg/ml which is 5-25ng/dl which is close to the free test ranges of most labls. And Dr. Abraham Morgentaler says > 15 ng/dl I’ve read his book. But seems the author has 1.1! Even the primary care told him he is low.
If he tests albumin and SHBG the puzzle with his free test will be completely solved.

My free t is measured in pg/mL also and I am at 25.7 last check, which is above range (8.7-25.1 pg/mL). It could be the mis-type of labcorp that is said to be wrong but I would think if it was truly wrong though and pointed out by doctors it would have been corrected by now, which it hasn’t. LIfe extensions also says the ideal range is 20-25 pg/mL, not ng/dL.

Hm strange maybe this is due to the measurement method I have no other explanation.

Thats why most progressive doctors use calculated free t, because it is unified measure

Problem in trying to help you is that we cannot tell if your FT is high or low. Also, cant really tell a whole lot about your E2 either, as SHBG wasn’t tested.

I believe E2 should be read in conjuction with SHBG in order to tell if that E2 is bound or not. The lower your shbg is, the less bound your E2 is, and therefore could present as more E2 symptoms…

I understand and I will update you all when I get more bloods done (soon). What I do not really understand is the confusion surrounding my Free-T number, why there was no reference range on the results, and exactly how low this number is. Thanks

I was skimming, my mind is slipping the longer I’m off TRT.

It’s basically useless without reference range. I would call dr. and have him figure it out. I mean, you PAID for this test.
