Muscle Pain/Twitching Due to Low T?

I would like to pose a question on symptoms of muscle pain, DOMS and muscle twitching that are potentially due to low testosterone levels. I need to go into my medical history a bit, but I try to make it brief.

Two years ago I had one of my testicles surgically removed on suspicion of testicular cancer. I had pain in my testicles and the urologist saw a black shadow on the ultrasound and two days later the surgery was done. There was no blood tests or anything before the surgery. In the end, it was not cancer but a hematoma - I was relieved but never really got an explanation for this mis-diagnosis.

About half a year later my ejaculate was reduced in volume and changed consistency (more watery) so I saw the urologist, who did another ultrasound of the remaining testicle, said everything was fine and wanted to send me away. I insisted on a testosterone test, but was never contacted on the result, so I assumed everything was OK.

My sex drive was and is very low, but I was/am a father of two very young girls/babies so I assumed it is just due to the stressful living situation of a young family. I was/am also quite low on energy, but put it also down to my stressful living situation.

A year after the removal of the testicle, I started to have muscle pain in my right thigh and sporadic muscle twitching all over my body. I googled it and landed pretty quickly on ALS which obviously freaked me out quite a bit. I had a lot of bloodwork done and went to a neurologist who did a thorough check-up and said he sees no indication of a neurological problem and send me on my way.

The muscle pain never went away in the last year and spread to the lower leg(s), my righ shoulder, and both hands. It is very pronounced, after I do strenuous activities - like intense DOMs. I also noticed the affected muscles getting softer and smaller. The twitching also never went away but also did not increase in intensity.

About half a year ago I remembered the testosterone test and called the urologist. I had a total testosterone of 290 ng/dl (10 nmol/l). I did some follow-up tests which were at 267 and 262 ng/dl (9.1 nmol/l) and also a free testosterone test that was at 6 ng/dl (208 pmol/l).

I was having a hard time to find a gp/urologist/endo to do something about it (since it was still in the “standard range” in my country) but one of them finally started me on Nebido 1000mg TRT two days ago as a trial run (no real effect yet).

Now back to my question: do you think my (comparatively low) testosterone levels could cause my muscle pain and twitching - considering that it is probably quite a bit lower than what I had before the removal of the testicle (unfortunately I have no test-results from before). If so - when would you expect it to clear up with Nebido 1000mg TRT?

I stopped TRT in April and in the last month I’ve been having muscle twitching throughout my body just like pre-TRT. It goes on all day, but is worse at night time. The legs are the worse.

There could also be an electrolyte imbalance. The nebido is the worst drug out there for TRT, the half-life is long and will take a long time to reach stable levels. The half-life is so long that a lot of men just don’t do well on it at all.

Forgot to add: I am currently 37 years old, 178 cm tall, weigh around 98 kg (maybe slightly overweight) and was quite burly/strong for someone who doesn’t do any weightlifting. As I said, my muscles have started to get smaller and softer recently.

Mine also, the softer they get, the worse the twitching.

Do either of you supplement with magnesium?

My mag levels are above mid-range without supplementation. My mag RBC is at 5.8.

Maybe you need more? Just a thought.

I do supplement with magnesium citrate ever since the twitching started. I feel like it really reduced the twitching but not the muscle pain (which is the real problem) - the twitching is just scary and inconvenient.

@systemlord do you have muscle pain as well? What is your testosterone level pre and post TRT?

Yes I do have muscle pain. TT is 97-146 in the last couple of months and FT very low, however I feel great at 400 ng/dL, FT 15 (ranges 6.8-21.5). I have low SHBG, so I have a lot of FT in circulation.

I need very little to get me optimal, 7mg daily. If I started 7mg daily tomorrow, by the end of the day I will be feeling good and better sleep by day 2.

Things happen more quickly on cypionate because the half-life is 7-8 days versus nebido is over 30 days.

Can you elaborate on the reason? I just bought a large supply of citrate.

Citrate is for stomach issues like constipation. If you need to take a shit then citrate is perfect for that. Glycinate is for dealing with stress, sleep, anxious feelings and can help with things like inflammation. There’s a very big difference that you can feel.

I think Nebido is quite common where I am from (Germany) - I did not really know anything about it and also did not question it. Do you think it would have an impact on my symptoms after just one injection/cycle? I have an appointment with my endo mid of January to discuss if it helped and whether I should continue with it / TRT.

I am inclined to continue with it no matter what, based on my testosterone levels alone. That said, yours are/were quite a lot lower than mine.

No, not likely. You probably have other serious issues that need to be addressed. I had those low numbers for years. I lifted 3-4 times a week and was a competitive Strongman. Never had DOMS, ever. I have it all the time now with normal test levels and lifting.
Your symptoms are something else, possibly something causing the low test.

Cypionate is the most widely used ester in the US, enanthate is number 2 (2 day half-life difference), sustanon comes (multiple half-lives) in at third place and nebido last place with the longest half-life. Sustanon is more common in the UK, however some are using enanthate in the UK and some are importing cypionate.

Impossible to answer, some are hyper TRT responders, some average responders and those who take a little longer to notice improvements and others who feel worse and those who just feel nothing at all.

How is that possible? People supposedly wait months for TRT to show results. Is low SHGB really such a drastic factor? I see horror stories of people with low SHGB aromatisting like crazy even on 50mg cyp e3.5d.

My doctor has seen this before, someone who responds very quickly and it has to do with having shorter length AR gene CAG repeat numbers, in other words very sensitive T receptors.

There was a guy in Canada who was on standard dosing and nothing crazy and his E2 was in the 400’s. I’ve seen super lean muscle men aromatise like crazy. I don’t aromatising a lot when on daily dosing as long as I stay around 400 which is more than enough considering the awesome results happen weeks before I reach this number.
