I think it is.
The guy’s got talent either way. If it’s real, he’s very talented to be able to produce such an amazing result from such a basic tool as MS Paint.
If it’s a fake, he’s good a producing a convincing fake.

Even if he worked backwards and used a reference we never saw, that’s some skill!
