I’ve been on the HRT pellet program for almost two years. At first I felt great, getting estradiol and tested one pellets. Due to breakthrough bleeding they eliminated the estradiol and put me on 900 mg of progesterone. I’m on testosterone pellets with 450 mg of progesterone at night. I’m tired all the time and gaining weight, no energy. My testosterone was 296 and my estrogen was 23 last labs, just had labs done 3 days ago. Is my test to high and my E to low? I need help because I’m not getting it. Thank you
Testosterone? Probably, need free testosterone to be sure. Estradiol? Certainly.
Are you taking 900mg progesterone, or 450? Do not take estradiol unopposed. Are you tired in the morning? Could be a sign of too much progesterone.
A better TRT regimen would be 5 mg cypionate twice weekly. Also anything more than 10 mg weekly is too much for most women.
I’m actually heard women’s accounts of going from pellet therapy to twice weekly injections and they rave about it!
That’s hard to say without checking the Free T, but this would be optimal for some women.
Your estrogen is low.
No reason to do twice weekly. Very rare for women to notice fluctuations with once weekly. I have two, out of over 400, that inject twice.
Edit: It occurred to me that around half of them use a cream. So, it is two out of around 200.
Not really, easily more than half take 12-18mg. I did have one doing very well at 80mg however.
Relax everyone, she misloaded the syringe, was supposed to be taking 8mg. I understand I am fairly conservative compared to others. Did see one who was taking 200mg, prescribed by another doctor. Six months on, yeah, that wasn’t working out, hence the switch.
Does this have any relation to pre and postmenopausal status?
Good question, I do not know. I don’t think so, but I’ll pay attention to age and see if there is an association.

I don’t know what my free T is but hope to have my labs back by the middle of the week. I am now on 450 mg of progesterone but when I started it was 225, it’s all over the board with me. Thank you for your reply
I do not think there is a one-size-fits-all ‘optimal balance’. What feels good for you may or may not feel optimal to others.

Are they planning to put you back on Estradiol?
For me it is post menopausal. I have been in the progesterone since going in the program nightly. I started at 225mg, went to 450mg due to breakthrough bleeding, then 900mg to stop it then back down to 450. I asked about going back on the estradiol twice but haven’t seen the doctor just his assistant and the PA so no answer until they see my labs, hoping any day now
I got my lab results today. My testosterone is 298, my estradiol is 25, my free test is 4.5 and my progesterone is 2.6. Anyone have any suggestions on why I feel like total crap? No energy, tired all the time and gaining weight.
Yeah, your estrogens is in the gutter. You would expect to see estrogen below 30 in a premenopausal woman.
Men and women with low estrogen will gain a lot of belly fat. Fatigue is common to both sex with low estrogen, just ask any man that’s crashed his estrogen using anastrozole.
Labs obtained from LabCorp? You likely need more testosterone. As mentioned above, you certainly need more estradiol. I’d increase progesterone as well.
What do you think my testosterone and estradiol should be? I was on 900 mg of progesterone when my Estrogen was at 60 due to breakthrough bleeding, hence tired all the time. They can’t find an optimal for me.
If you are pre menopause do not take Estrogen. Progestrone should be taken as needed, its a feel good hormone for woman.
Pellets is bullshit. get the cream and apply to the surface of the V at night. You are being mismanaged.
If you are Tired all the time you want to check your thyroid.

Free testosterone high enough to relieve symptoms, usually 5-15 (free T) does it. Estradiol usually between 80-110 and progesterone 10-20. Guessing much of the fatigue due to bleeding?
My free t has been at 4.5 for over a year, it is the others that bounce around. The progesterone used to make me tired, now just tired all the time. No energy at all.
My wife is 47 and on 18mg of Test Cyp. a week. I just had her check her latest bloodwork. Free T was 4.5. Estradiol was 79 and Progrsterone was 11. She feels great and falls right in line with the numbers that you gave.
