-describe body and facial hair: Grows fast generally hairy

-describe where you carry fat and how changed: Had a mild gut, lots in my face and neck, gone by losing 40lbs on a long term cut.

-health conditions, symptoms [history] - Familial hypercholesteremia (untreated, no heart disease)

-Rx and OTC drugs, any hair loss drugs or prostate drugs ever : none

-lab results with ranges:

T Total (quest) = 997 ng/dl(250-1100 range)
Free T = 94.3 pg/ml (35-155) range.
E2 = 20 Range <39
SHBG = 54 (10-50) This one was high

-describe diet [some create substantial damage with starvation diets]: High protein, usually calorie controlled to be under 2500 a day.

-describe training [some ruin there hormones by over training]: Full body weights 3 times a week.

-testes ache, ever, with a fever? : No

While my total T is on the high side of normal, my free is right down the middle and I’m guessing, perhaps wrongly, this is due to the high levels of binding hormone. I’m wondering if there would be any benefit to lowering E2 or the like.

Hi im not an expert but also have high SHBG which i think is strangling the life out of my T. I have a suspicion it is to do with the epilepsy meds i have been on for over 12 years as when tested my E2 did not seem to be astronomically high so cannot put it down entirely to that. I am starting a liver cleanse using milk thistle to help kick start my liver to hopefully lower my SHBG and free some T.

If i read it correctly your E2 is 20 which seems close to optimal from what i’ve read so maybe look into the condition of your liver and other possible factors before potentially crashing your E2 too low.

Your high protein diet is spiking your SHBG. Go on a donut, candy bar and soda pop diet. Once you develop insulin resistance, your SHBG will drop. The diabetes is just a side effect of high free T.

I do not know what to say in a case like this one. SHBG is make in the liver to allow your body to clear hormones from your blood stream. More E2 typically means more SHBG. But E2 seems to be a very reasonable level. You could try some AI and see what happens. If SHBG goes down, then T production may increase and then there will be more T–>E2 and E2 levels may not decrease too much.

Something might be triggering SHBG production.
Alternatively, your liver is not clearing T+SHBG from your blood.
Liver labs ALT/AST might be useful, also CBC

You might have labs for E1, E2 and E3 to see what is going on there. High progesterone would be very unexpected unless your wife uses such transdermals. Any OTC or Rx drugs? Do you apply any products to your skin? Cannabis can mess with hormones.

Check liver.

Not really much you can do. Maybe milk thistle can help?

E2 looks great.

If still having low T symptoms you could aim to increase free T to near top of the range and disregard total T.

Some additional information.

On my first test, my total estrogen was 316pg/ml when it should be under 130. This is obviously hugely high, but I have absolutely NO signs of high E. When I got to the doctors office for the first time she was expecting me to be obese with man tits. She actually copped a feel to see I didn’t have tits. She was surprised. Personally I think it was a bad test result, which is why I insisted (she wasn’t going to retest) for E2 but oddly she didn’t redo Estrogen even though she thought it might be a bad test as well.

The good news is she is one of those who think hormones should be on the high side of normal, the bad news is she is one of those bioidentical hormones only type of docs which is good for my wifes needs but everything else is “another drug” to her.

Also on my first test my total T was 557 (range 241-827 ng/dL) but Quest screwed up and forgot to test for Free T.

Honestly I don’t have a clue and I can tell I’m out of my docs depth on this one, she deals mostly with women.
