Hey everyone. Saw this forum today and thought why not ask you guys for some help.
imma 19 year old asian …been lifting for about 3 years with less gains and more bodyfat…about a year ago i started dirty bulking consuming about 4500 calories cuz i couldn’t see any gains before…my weight increased about 20kg …then i started cutting to decrease the bodyfat but in 3 months with proper training and nutrition i lost more muscle than bodyfat…i got my T levels checked because i had almost all the symptoms…visited an endocrinologist who considered them normal…hardly found one who was ready to work about it…Following are my test results…Would really really appreciate anyone’s help …im 19 and dont want to waste more years 
Total testosterone= 360ng/dl 250-1100ng/dl
free testosterone= 8.96pg/ml 1.88-21.48
FSH=1.48 mIU/ml
LH=1.39mIU/ml
HBsAG=negative
T3=82.63ng/dl 58-159
T4=6.16 (micro)g/dl 4.87-11.72
TSH=1.29(micro)IU/ml 0.35-4.94
Estradiol= 11pg/ml 11-44 although i have gyno!
ALT=20U/L <41
Prolactin=15.99 in the first report where doctor gave me bromocriptine since levels were high…in the second report after 20 days they were about 1.49(below normal)…definitely something wrong with the first report
My vit D came in as mild deficiency so i am currently using 50000 IU pr week
PLEASE SOMEONE HELP
Medications? Head injury? Testicular injury? Steroids?
Your 19 years old. Must investigate thoroughly.
Probable need a pituitary MRI since FSH so low.
Testicular ultra sound.
Full labs.
Lipid panel
Iron panel
Ferritin
Cortisol
Acth
Complete metabolic panel
CBC
Repeat testosterone labs taking within 1 hour of getting up from a good night sleep.
If doctor does not think you need to investigate further find another Endocrinologist or can try a urologist.
Most men start having problems when bioavailable testosterone gets near 10 pg/mL, your total testosterone being in low normal but in range while free testosterone is clearly low.
No SHBG testing, this test would have shown how well you hold onto your testosterone and affects your free testosterone levels. Sadly these ranges do not account for age, it’s not alright for a 19 year old to have similar testosterone to a 80 year old man.
It will be difficult to locate a doctor willing to shut down a 19 year olds HPTA, LH and FSH are very low. Far from normal.
Do you have ranges for prolactin?
it was my first check up with this endocrinologist.
do you think i should go for another one?
no injury no steroids…i’ll get my testicular ultrasound this weekend because doctor too asked for it…and the free testosterone,lh fsh tests were taken at evening …i did ask my doctor about the timings but she said go for these tests rn and other ones later.
the normal range for prolactin was 2-15 for the first test and 4-19 for the second test
Thank you guys for replying. Please do tell me if this is really something to worry about because doctor told me no researching regarding all this since my T levels might be low due to stress or something…is TRT the only solution?at this age it might give side effects later?
It’s too early to diagnose anything. It would be rare if you require trt.

Take labs in the morning within 1 hour waking up.
You went through puberty fine?

and one more thing…doctor asked me for the SHBG test too…but her lab gave me results of HBsAG test saying they are same things!!!
No medications except bromocriptine which my first doctor recommended because he thought i had high prolactin levels, and Vit D medications because i am deficient.
with the puberty thing i guess its fine…i am 6ft 2 with some facial hairs…i dont get much erections these days compared to a year ago
You need to run labs before 10am, no endocrinologist with half a brain would have you draw labs in the evening. Prolactin barely over the range is hardly anything to worry about, doctors see out of range and freak out.
HBsAg is the surface antigen of the hepatitis B virus, SHBG is Sex Hormone Binding Globulin (produced in liver) and not even remotely the same thing.
When my testosterone was low I to had a Vit D deficiency which corrected itself within months of starting TRT. Insurance doctors will struggle diagnosing hormonal issues, incompetence is around every corner when TRT is concerned.
Few doctors know how to diagnose low testosterone and fewer know how to design a modern TRT protocol based on your SHBG value. I was forced to go private because all my endocrinologists were lazy and uninterested in learning how to do TRT, it was like they had never done TRT before and everything they suggested was at random.
None of then had any knowledge at all as it pertained to TRT. If you continue to have trouble with your doctors, you might consider Defy Medical. Defy staffs experienced hormone specialists and do telemedicine.

Hey mann…got my SHBG results today…they are 22.3nmol/L range:14.7-44.9
are they normal?
That was my pre-TRT SHBG level, after going on TRT it dropped to 16 nmol/L, excess androgens lower SHBG in most men unless the reason for SHBG being lowered is do to diabetes or insulin resistance, in which case some see an increase in SHBG.
Your SHBG level doesn’t set off any alarm bells, any TRT protocol needs to focus on frequent injections, every other day would be perfect for you.
