Low Free T, High Albumin/SHBG and Low LH/FSH. Blood Results a Cause for a Concern?

I am 22 years old European male, recently been on a blood test and I’ve received results from which I am confused.

I have started with weight lifting and better eating habits approximately 3 months ago, as I was fasted and barely eating. Went from 76 to 85 kilograms ( i am 183 cm tall ).

As I understood from these tests, I need to get more vitamin D and raise the free T by lowering Albumin & SHBG.

However, what is concerning me is the fact, that even at this age I am still unable to develop any facial hair ( only moustacche and slight pubes at the sides, next to ears, I mean no beard or stubble at all ), nor any signs of chest hairs.

I also have big dark bags under eyes, for almost my whole life, that I remember.

  • I also feel tired often.

Do these results provide any signs of some possible disease or some hormonal imbalance?

Albumin 52.20 g/l [ 35 - 52 ]
SHBG 51.50 nmol/l [ 18.3 - 54.1 ]

Free T 0.347 nmol/l [ 0.243 - 0.571 ]
T 22.93 nmol/l [ 8.64 - 29 ]

Vitamin D 26.5 ng/ml [ 30 - 100 ]

LH 2.68 mIU / ml [ 1.7 - 8.6 ]
FSH 1.65 mIU / ml [ 1.5 - 12.4 ]

Check liver functions. Its included in the metabolic panel. Also lipid panel. And CBC. Also check prolactin, and thyroid panel.
Take DHEA-s and DHT and estradiol sensitive.
High shbg and albumin can indicate liver issues

What about the FSH? I know it is " in range ", but isn’t it very low for a 22 yo male?

It’s on the low end but hormones can fluctuate by alot during the course of the day. Retake FSH and LH. Your testicles are definitely getting the signal and producing testosterone. Like you said the problems is the free t.

Take your labs in the morning within an hour of getting up.

I added 2 more test to take. Dht and dhea-s. Dht has to do with hair growth.
Also state any medication you take.

I do not take any medications and I’ve forgot to state, I never did steroids.

Also, these results are recent ( 5 days olds ) and have been taken in the morning, an hour of getting up.

Start reading about high albumin levels.

Ok, now i’ve found out, that higher albumin can be from dehydration - I didn’t drink any water from the time I woke up until the blood test ( simply forgot ), otherwise I am drinking tons of water ( I am taking Creatine too, so its a must )

Always drink water before blood test. Must be hydrated. Also Apparently high protein intake can affect albumin. Maybe some diet changes for a week then redo labs as stated.

You just want to get best assessment possible on labs before deciding on treatment.

I am thinking to start taking Vitamin D, Zinc, Boron, Fish Oil, B12, Folic Acid and Magnesium.

( D for defficiency, others to raise the free T ), whats your opinion on this?

SHBG is very high for a young man, it’s binding up the majority of your T and choking Free T, some men have low Vit D when testosterone is low. Your Total T is unavailable to your body, you rely on Free T and it’s low.

The high SHBG is inflating your T status making it appear high when in fact it’s high because Free T is low. LH is a better window into your testosterone production, low LH and low T follows.

TRT will lower SHBG, androgens lower SHBG. Dark bags under eyes is known of those with liver problems, it just so happens SHBG is too high for such a young man and is expected later in life, you should look further.
