Low E2 with High Free T -- Thoughts

So have been on TRT for about three years.
I took a recent test about two weeks ago as I was starting to feel very different. Extreme fatigue, no libido, headaches, dont feel like myself. And have been on 50mg enanthate twice weekly for a while now, dont take anything else other than mens prostate multivitamin and some vitamin D. Any thoughts about what is going on or what i can try different.

Total Testosterone 697.4 ng/dl
Free Test 24.13 ng/dl Flagged High
Estradiol Sensitive 9.4 pg/ml
DHEA 513 ug/DL Flagged High
Dihydrotestosterone 72 ng/DL Flagged High

And you’re not taking any AI’s? Yikes.

Injecting more frequently tends to lower estrogen, have you tried injecting 100mg 1x/wk before? Are you injecting IM or SubQ?

Here’s your problem, low E2 causes headaches and low libido.

Free T, which you have plenty of, is converted to E2 and you’re not aromatizing much.

You may have an aromatase deficiency. HCG might help, or estrogen replacement.

No AI’s i have them from the pharmacy but havent touched in over a year. I may try to do one large dose for my next dosage and do IM in the delts always.

Yes, I may try to add some HCG in the next few weeks – is 750-1000iu weekly split in two enough from what ive read.

IF you lower dose with it.

If not, it will INCREASE E2.

So you’re saying that injecting 100mg 1x/wk would have less aromatase activity as 50mg 2x/wk or 14.3mg ED? Trying to clarify

Kind of.

The main difference is that more frequent injections will reduce the gap between peak and trough levels, your mean levels are probably the same.

So if you inject once/week and test your E2 on the 8th day in the morning, you will most likely show lower levels than if you inject daily and test your E2 24 hours post injection. That’s all that’s going on, it’s pretty simple but for some reason almost no one seems to understand that.

Its amazing how many doctors overlook estrogen as if it is totally irrelevant to the male hormone profile. My E2 came back as undetectable! Talk about the most horrendous experience. I totally understand the horrible exhaustion and the total lack of interest in sex. Currently trying to raise my E2 level also without throwing everything else off balance.

That was my experience, yes.

Unless i decreased my dosage when increasing frequency, E2 was greater.

It will obviously depend when you’re testing (when testing the less frequent shot protocol).

YMMV.

I always figured this. If you checked e2 every day on a 1x injection schedule and then checked again on the EOD injection schedule you’d see that the average for each was pretty close. You’ve just lowered the highs and raised the lows, but overall if dose is the same there isn’t much difference
