I am a 29 year old man, with all the regular symtoms of low test levels. Memory fog, libido, drive, depression, and fat around my chest and stomache that wont go away… the whole shabang. Ive been to few endos and constantly bothered my doctor about it. They all ran the test, and i always came up in the low normal range and ofcourse said i was fine, disreguarding all my symtoms.
Well that leads me to my newest quest for answers. I found a dr about 4 hours south of me that was reguarded as one of the top experts in biodentical hormone replacement. So i made my appointment and took the trip. Here is the results for the first round of testing.
He said the results look good, but i informed him that i wasnt feeling any better. With only the slightest of improvements. Which kind of puzzled me. He basically told me if i had any suggestions to let him know, which is very disheartning, as ive spend so much monry and time looking for answers, all while feeling like dog shit, and here is asking me to figure this out. So he told me to keeping taking it for 6-8 more weeks then we would lower the dose and taper
Sorry for ranting abit, just need to let off some steam.
If anyone has any suggestions, please advise me. Im seriouly suffering here, i feel like a little old sickly man, trapped in a 29 year old body.
Your numbers are greatly improved.
Total and Free look good, your E2 is still too high, and it’s the thing
making you feel tired, old and is responsible for female fat distribution.
You need slightly more Adex. Do you have enough to bump up to 1.5 per week ?
If so, take .5 m-w-f . You want E2 in between 20-30. It should make you feel better,
but will not be a magic formula to lose weight. It will help, but it’s no magic bullet.
I don’t have first hand experience with SERM (clomid etc) restarts, but I believe that
they lose effectiveness after a while. You can search the forum and read about
other guys that have gone through it. Hopefully some will read your thread and
comment as to their experience.
Thanks for the advise. I will look into raising up my AI. From what I understand tho, you cant get accurate estrogen testing while on a serm. Is this not true ?
SERM does not prevent good labs but does increase E2.
You need to be on an AI or you will get HPTA shutdown “estrogen rebound”.
Stay on the SERM, then taper slowly. Take 1.0mg anastrozole per week in divided doses and taper down to 0.5 per week and cruise on that.
Your cortisol looks like its the bottom end of acceptable. Try low dose cortisone and see how you react. This trial will be diagnostic.

Your TSH says something is wrong, the reference ranges are useless, best to be near 1.0
You were estrogen dominate and with the SERM, E2=40 is also a house of pain
Anastrozole probably will not lower E2 while you are on that dose of SERM, LH is too high promoting very high T–>E2 inside the testes and the intratesticular T levels may be 100 time higher that serum and anastrozole cannot work there.
Suggest that you spend some time at 1/2 your clomid dose so AI can work
Some guys feel like shit on clomid because it activates estrogen receptors for some. So this might also be at play. I always suggest Nolvadex because it does not do that.
There is a lot above to deal with, lots of reading and study, you need more diagnostic lab work as suggested. There are thyroid issues that you can deal with on your own now, temperatures and iodine. Do that, then you can rT3 labs if temps are lowish.
Ksman i apologize for making a new thread, i couldnt seem to find this one. I will have my prolactin levels checked when i see my doctor next week.
As for the rest, i will spend some time going over everything and take my tempatures and report. I do seem to get rather cold easily and hot easily.
Im going to number a few questions amd answers to keep abit of structure as this is alot to take in.
My iodine intake is very underwhelming, its definitley not something i purchase and go out of the way to use in my food. I take a vitamin called optimen, i will have to check out the ingrediants.
Would having cortisol levels on the bottom End of acceptable be suggestive of adrenal fatigue? I do use topical cortesteroids for skin issues sometimes, and spend 5-7 hours a day on the computer for work. What type of method is there for replacement?
As for the SERM. I actually have been on adex 1 mg a week during the restart. Ive been taking clomid for 10 weeks. I have my e2 down to 32. Is this still to high?
I notice that i sweat very easily during excersion in any that isnt very cold or comfortable.
I have taken klonopin/ xanax for almost 9 years and have now been off for 3 months. From what i understand, this might be caused/ causing hypothalamic supression, and that it can take up to a year for things to recover and restart.
5, i wil ready more into the thyroid thread. So having a high tsh is suggestive of under or overactive throid function?

Follow what the doctor says and don’t make changes on your own. You should be more patient. You are just starting and it often takes a while for things to get tuned and even after that for benefits to kick in.
